Adult Congenital Heart Disease: The Complete Guide
If you were born with a heart defect, your story probably started with surgeons and children's hospitals. As an adult, it continues in a world that isn't always built for you: general cardiology that doesn't know your anatomy, insurance forms, jobs, relationships and the quiet question of what getting older will look like.
This guide brings the whole picture together and links to a deeper article on every part of life with a congenital heart.
Here is the short answer. Most children born with heart defects now reach adulthood, but a repaired heart usually needs lifelong care. Valve leaks, rhythm problems and heart failure can develop silently over decades, about four in ten adults drift out of specialist follow-up, and acquired risks grow with age. Staying connected with an ACHD clinic and working on the same shared root as heart attack survivors gives the best long-term outlook.
That shared root, oxidative stress and chronic low-grade inflammation, links congenital hearts with heart attack and stent recovery (see one inflammation, three hearts).
How many adults live with congenital heart disease?
Congenital heart defects affect about 1% of births, and thanks to surgical success most of those children now grow up. About 1.4 million US adults live with congenital heart disease (CDC), and in Quebec adults made up about two-thirds of everyone with the condition by 2010 (Marelli et al., Circulation, 2014). This first large generation of adult survivors is now reaching midlife (see growing older with congenital heart disease).
Why a repaired heart needs lifelong care
"Fixed" usually means "repaired". Many repairs leave changes that can evolve over decades: leaking or narrowed valves, stretched chambers, scar tissue that triggers arrhythmias and, in a Fontan circulation, effects on the liver. In a large contemporary cohort, chronic heart failure was the leading cause of death in adults with congenital heart disease (Diller et al., Circulation, 2015).
Yet about 42% of adults have had a gap in cardiology care of more than three years, usually starting around age 19 or 20 (Gurvitz et al., JACC, 2013; see the ACHD transition gap). Specialist care matters: greater referral to specialised ACHD centres was associated with lower mortality (Mylotte et al., Circulation, 2014). The composite story in "I was fixed at age three" shows how this plays out.
Daily life: energy, exercise, mind, work and family
Energy: fatigue has layers, from heart output to iron and sleep (see fatigue in congenital heart disease).
Exercise: most adults can and should exercise with an individual prescription (see exercise with congenital heart disease).
Mind: about half of patients in one ACHD clinic met criteria for a mood or anxiety disorder, many rooted in childhood hospital experiences (see childhood medical trauma and scars and identity).
Work and insurance: you usually don't have to disclose a diagnosis, and you're protected if you need accommodation (see work and insurance in Canada).
Family: recurrence risk is usually a few percent, and planning ahead protects your own heart too (see family planning and genetics).
Seasons: vaccines and a sick-day plan matter every fall (see vaccine checklist).
The shared root, and what helps
Repeated surgery, a heart that has worked harder for longer and acquired risks with age all feed oxidative stress and chronic inflammation, and higher inflammatory markers have been linked to worse outcomes in adults with congenital heart disease (see ACHD and inflammation). The same daily foundations that protect heart attack survivors help here: movement within your team's guidance, sleep, stress care, a whole-food eating pattern within any fluid or sodium limits, not smoking and knowing your blood pressure, cholesterol and blood sugar. What feeds that inner cycle comes from outside: the way we eat, move, sleep and drink, our environment, daily habits, stress and mind-body balance, relationships, how well we understand and follow our care, and our sense of purpose. The Healos™ H-Index groups these into 11 categories, which is why it looks at 32 dimensions of everyday life rather than a single number.
Healos™ supports those foundations between ACHD visits. The 91-Day Protocol moves through Clear, Replenish and Repair, providing complementary cellular nutrition, with check-ins at days 30, 60 and 91 that track energy, activity, sleep and mood. Sovereign Monthly · Protect continues that support month to month for lifelong maintenance.
ACHD special considerations
Many adults with a Fontan circulation or heart failure have a fluid limit. Many take anticoagulants, so anything new needs a pharmacist check. Endocarditis prevention matters for many repaired hearts. Rhythm problems, cyanosis and pulmonary hypertension change what exercise, nutrition and pregnancy planning should look like. Your ACHD team's guidance overrides any general advice, including ours.
The bottom line
A congenital heart is a lifelong relationship, not a childhood chapter. Stay connected with an ACHD clinic, watch for slow changes, look after the shared root every day and plan ahead for work, family and the years to come. Use the library to go deeper into each part of life with a congenital heart.
