  • "I Was Fixed at Age Three": When Congenital Heart Disease Returns in Adulthood

This is a composite case, built from common experiences of adults with repaired tetralogy of Fallot. "Maya" is not a real patient, and details have been combined for illustration.

Maya had heart surgery when she was three. She doesn't remember it, only the scar and the yearly hospital visits that followed. At 18 she moved across the country for university, and the visits stopped. "I'd been told I was fixed," she says. "I didn't think about my heart for fifteen years."

At 33, the stairs at the subway station started to feel harder. Then came occasional fluttering in her chest at night.

Here is the short answer. Many people repaired for a congenital heart defect in childhood were told they were "fixed", but most repairs leave changes that can progress silently for decades. Maya's fatigue and palpitations led, after years without follow-up, to a diagnosis of a leaking pulmonary valve and a plan for valve replacement. The lesson: new symptoms deserve a specialist look, and it's never too late to reconnect.

Her story also has a thread common to many heart recoveries: the long-term biology she now manages, oxidative stress and chronic low-grade inflammation, is the same shared root heart attack survivors work on (see one inflammation, three hearts).

Why a repaired heart can change in adulthood

Tetralogy of Fallot is repaired by closing a hole between the ventricles and widening the outflow to the lungs, often by enlarging or removing part of the pulmonary valve. That surgery saves lives, but it frequently leaves the pulmonary valve leaking. Pulmonary regurgitation is the most common long-term problem after tetralogy repair (Geva, Journal of Cardiovascular Magnetic Resonance, 2011).

For years, the right ventricle copes by stretching. Eventually it can enlarge and weaken, and the stretched muscle and old surgical scars can trigger arrhythmias. Because the change is slow, many people adapt without noticing, until something like Maya's stairs makes it obvious.

Her fifteen-year gap is common. About 42% of adults with congenital heart disease have had a gap in cardiology care of more than three years, usually starting around 19 or 20 (Gurvitz et al., JACC, 2013), and a lapse in care has been linked to about three times higher odds of needing urgent intervention (Yeung et al., International Journal of Cardiology, 2008; see the ACHD transition gap).

Maya's path back: from family doctor to ACHD clinic

Step 1: Saying it out loud. Maya mentioned the stairs and the fluttering to her family doctor, along with the old surgery she'd almost forgotten. That history was the key detail.

Step 2: A referral. Her doctor referred her to an adult congenital heart disease clinic. Maya asked her parents to find her childhood surgical records, which helped the team understand exactly what had been repaired.

Step 3: The tests. An echocardiogram showed significant pulmonary regurgitation. A cardiac MRI measured the size and function of her right ventricle, the measurements that help guide timing of valve replacement. A Holter monitor recorded short runs of extra beats. An exercise test showed her capacity was lower than expected for her age.

Step 4: The plan. US guidelines recommend pulmonary valve replacement for adults with repaired tetralogy of Fallot and moderate or greater regurgitation who have symptoms (Stout et al., Circulation, 2019). Maya's team discussed two options: surgical replacement or, if her anatomy suited it, a transcatheter pulmonary valve delivered through a vein. They also set a follow-up rhythm she could keep.

What Maya wishes she'd known

  • "Fixed" usually means "repaired", and repaired hearts need lifelong check-ins, even when you feel fine.

  • Slow changes are easy to miss. Comparing what you can do now with a year ago is more revealing than how you feel today (see fatigue in congenital heart disease).

  • Your childhood records matter. Request a summary while they're still easy to find.

  • It's not too late. Most problems found in adulthood can be treated well; the earlier they're found, the more options there are.

  • The years between visits count. Movement, sleep, stress and steady medication habits support the heart you have now (see growing older with congenital heart disease).

ACHD special considerations

Every repaired heart is different, and Maya's story won't match everyone's. Many adults with a Fontan circulation or heart failure have a fluid limit. Many take anticoagulants, which need a pharmacist check before anything new. Endocarditis prevention becomes even more important after a valve replacement. Rhythm problems and cyanosis change what exercise and nutrition should look like. Your ACHD team's guidance overrides any general advice, including ours.

How Healos™ supports adults with congenital heart disease

Healos™ is built for the stretches between specialist visits, the part of Maya's story that went unwatched for fifteen years. The 91-Day Protocol moves through Clear, Replenish and Repair, three sequenced stages of complementary cellular nutrition and lifestyle support, with check-ins at days 30, 60 and 91 that track energy, activity and sleep, so slow changes become visible. Sovereign Monthly · Protect continues that support month to month and can be cancelled any time. Neither replaces your ACHD clinic.

The bottom line

Being "fixed" as a child is one of modern medicine's great successes, but it's the start of a lifelong relationship with your heart, not the end. If stairs feel harder, your heart flutters or your energy has quietly drained, tell your doctor about your childhood surgery and ask for an ACHD referral. Like Maya, you can find your way back.

Frequently asked questions

Can a heart defect come back after childhood surgery? The original defect usually doesn't return, but most repairs leave changes, such as a leaking valve or scar tissue, that can progress over decades. That's why adults with repaired congenital heart disease need lifelong follow-up.

Why am I getting breathless years after tetralogy of Fallot repair? A common reason is pulmonary regurgitation, a leaking pulmonary valve, which gradually stretches the right ventricle. Arrhythmias and reduced fitness can also contribute. Ask for an ACHD clinic referral for an echo and cardiac MRI.

When is pulmonary valve replacement needed after tetralogy repair? US guidelines recommend it for adults with moderate or greater pulmonary regurgitation who have symptoms, and cardiac MRI measurements help guide timing in others. Options include surgery or, in suitable anatomy, a transcatheter valve.

How do I get back into congenital heart care as an adult? Tell your family doctor about your childhood surgery and any new symptoms, ask for a referral to an adult congenital heart disease clinic, and try to gather your childhood surgical records.
