our heart, your career: disclosure, rights and insurance in Canada: What you have to share at work (often less than you think), how accommodation works and how to approach insurance.

Congenital Heart Disease at Work: Insurance and Disclosure

  • Work, Insurance and Disclosure With Congenital Heart Disease in Canada

Job interviews, benefits forms, a manager asking why you need every third Thursday afternoon off: life with congenital heart disease brings questions that have nothing to do with your heart's anatomy and everything to do with your career and finances.

Here is the short answer. In Canada, you generally don't have to tell an employer your diagnosis, but if you need workplace accommodation you'll share information about your functional needs. Human rights laws protect people with disabilities, including heart conditions, and employers have a duty to accommodate up to undue hardship. Insurance is more complex: group benefits through work are often the easiest route, while individual life and travel insurance require careful, honest disclosure.

Staying well at work also feeds into the shared root congenital hearts have in common with heart attack survivors: chronic stress, poor sleep and skipped appointments all add to oxidative stress and inflammation (see one inflammation, three hearts).

This article is general information, not legal or financial advice. Laws and policies vary by province and employer.

Do you have to tell your employer?

Generally, no. In most situations you're not required to disclose a medical diagnosis to an employer or in a job interview, and employers usually shouldn't ask about it. Exceptions can apply in safety-sensitive roles, such as some transportation jobs or positions requiring medical certification, where specific fitness standards exist.

Disclosure becomes relevant when you need something: time off for appointments, flexible hours during a procedure recovery, or changes to physical tasks. Even then, you typically share functional needs and limitations, often supported by a note from your doctor, rather than your full medical history.

Many people choose to tell a trusted manager or HR contact anyway, because it makes appointments and emergencies easier. That's a personal decision, and there's no wrong answer.

Your rights at work in Canada

Federal and provincial human rights laws prohibit discrimination in employment on the basis of disability, which includes many heart conditions. Employers have a duty to accommodate to the point of undue hardship, meaning they must make reasonable adjustments unless doing so would cause significant difficulty, cost or safety risk.

You're far from alone in needing this. In 2022, 27% of Canadians aged 15 and older had one or more disabilities (Statistics Canada, Canadian Survey on Disability, 2023).

Common accommodations for adults with congenital heart disease include:

  • Flexible scheduling or time off for ACHD clinic visits and tests.

  • Graduated return to work after a procedure.

  • Modified physical tasks, such as limits on heavy lifting if you have a dilated aorta.

  • Breaks, access to water and a cooler workspace, especially with a Fontan circulation.

  • Remote or hybrid work during recovery or flu season.

Your medical information should be kept confidential, usually with HR or occupational health rather than your direct manager.

  • 弹性时间或请假参加 ACHD 门诊和检查;

  • 手术后逐步复工;

  • 调整体力任务,例如主动脉扩张时限制提重物;

Insurance with congenital heart disease: what to know

Group benefits through work are often the easiest coverage to get, because basic group life, health and disability plans frequently don't require individual medical questions up to a certain amount. Read the plan details and enrol on time; late enrolment can trigger medical questions.

Individual life insurance usually involves medical underwriting. Outcomes vary widely by diagnosis, repair and current health: some people get standard rates, others pay more, some face exclusions or delays. Working with an independent broker who can approach several insurers, and having a recent summary from your ACHD clinic, often helps.

Travel insurance deserves special care. Most policies include pre-existing condition rules, often requiring your condition to be stable, with no new symptoms, tests or medication changes, for a set period before you travel. Answer medical questionnaires completely and honestly; inaccurate answers can void a claim when you need it most.

Genetic testing. Canada's Genetic Non-Discrimination Act (2017), upheld by the Supreme Court of Canada in 2020, prohibits requiring you to take a genetic test or disclose genetic test results as a condition of goods, services or contracts, including insurance. That can matter for congenital heart conditions linked to genetic syndromes.

Practical tips

  • Keep a one-page health summary from your ACHD clinic: diagnosis, repairs, current medications and follow-up plan. It helps with HR, insurers and travel.

  • Plan appointments into your work calendar a year ahead where possible.

  • Know your benefits plan: what's covered for medications, devices, mental health and disability.

  • Stay in follow-up. About 42% of adults with congenital heart disease have had a gap in care of more than three years (Gurvitz et al., JACC, 2013), and gaps can make insurance applications and travel coverage harder (see the ACHD transition gap).

  • Ask for help early: your provincial human rights commission, a union representative or an employment lawyer can advise if problems arise.

  • 保存一份 ACHD 门诊出具的一页健康摘要:诊断、修复、当前用药和随访计划,对 HR、保险公司和旅行都有帮助;

  • 尽量提前一年把复诊排进工作日历;

  • 了解你的福利计划对药物、器械、心理健康和残障的覆盖;

  • 坚持随访——约 42% 的先心病成人曾中断随访超过三年(JACC 2013),中断可能让保险申请和旅行保险更困难;

ACHD special considerations

Many adults with a Fontan circulation or heart failure have a fluid limit, which matters for long shifts and hot workplaces. Many take anticoagulants, relevant for physically risky jobs. Endocarditis prevention matters for many repaired hearts. Rhythm problems, devices and cyanosis may affect certain job requirements, such as some driving roles. Your ACHD team can provide documentation of functional needs when you need it.
