Medical Trauma From Childhood: Anxiety and PTSD in Adults With Congenital Heart Disease
The smell of hand sanitiser. The beep of a monitor. A blood pressure cuff tightening on your arm. For most adults these are neutral details of a medical visit. For many adults who grew up with congenital heart disease, they can trigger a racing heart, a knot in the stomach and an urge to leave, decades after the last childhood operation.
Here is the short answer. Many adults with congenital heart disease spent long stretches of childhood in hospital, and that experience can leave a lasting mark. About half of patients in one adult congenital heart clinic met criteria for a mood or anxiety disorder, most of them untreated. Medical anxiety can make people avoid the follow-up care they need, but it's common, understandable and very treatable.
There's also a shared thread with heart attack survivors: in both groups, fear and chronic stress keep the body's stress and inflammatory systems switched on, the same shared root that recovery works to calm (see one inflammation, three hearts).
How common are anxiety and PTSD in adult congenital heart disease?
More common than most clinics recognise. In a Toronto adult congenital heart clinic, about half of patients met criteria for a mood or anxiety disorder, and most weren't receiving any treatment (Kovacs et al., International Journal of Cardiology, 2009). In a study of 134 adults at a US adult congenital heart clinic, 11% had elevated post-traumatic stress symptoms specifically related to their heart disease or its treatment, and 21% had elevated symptoms overall, several times the 3.5% rate in the general population (Deng et al., American Journal of Cardiology, 2016).
Those numbers make sense. Many people had multiple surgeries before they could understand what was happening, long hospital stays, repeated needles and procedures, time away from family and school, and parents who were frightened too. Some memories are explicit; others are stored as body reactions without a clear story attached.
How childhood medical trauma shows up in adulthood
Medical trauma rarely looks like the dramatic flashbacks people expect. More often it shows up in everyday patterns, summarised in the table below:
Anticipatory anxiety in the days or weeks before an appointment, test or procedure.
Avoidance: postponing check-ups, not booking imaging, ignoring symptoms "because I don't want to go back".
Strong body reactions to hospital sights, sounds and smells, or to needles.
Hypervigilance about heart sensations, constantly checking the pulse.
Difficulty trusting health professionals, or feeling powerless in medical settings.
Low mood or exhaustion that's hard to separate from physical fatigue (see fatigue in congenital heart disease).
Why it matters: anxiety and the follow-up gap
The biggest risk of untreated medical anxiety isn't the anxiety itself. It's what it leads people to avoid. About 42% of adults with congenital heart disease have had a gap in cardiology care of more than three years (Gurvitz et al., JACC, 2013), and for some, avoiding the place where they felt most frightened as children is part of the reason (see the ACHD transition gap).
Gaps in care allow problems such as valve leaks and rhythm disorders to progress silently. Anxiety also makes it harder to sleep, exercise and stay consistent with medications, the everyday foundations of a healthy congenital heart. In the Healos™ framework, that's why mind-body-spirit state acts as a multiplier on every other part of recovery.
What helps: treatment and practical strategies
Medical anxiety and trauma respond well to treatment, and you don't have to have "proper" PTSD to benefit.
Talk to your ACHD team. US guidelines recommend that adults with congenital heart disease be evaluated for psychosocial needs, including anxiety and depression (Stout et al., Circulation, 2019). Some ACHD programmes include psychologists; others can refer you.
Evidence-based therapy. Cognitive behavioural therapy is effective for anxiety, and trauma-focused therapies, such as cognitive processing therapy, prolonged exposure or EMDR, help when memories or body reactions feel stuck.
Make appointments more manageable. Ask for the first or last slot of the day, bring someone you trust, write questions in advance, ask staff to explain each step before it happens and request numbing cream for needles if they're a trigger. Small changes in control make a big difference.
Calm the body as well as the mind. Slow breathing, regular activity and protected sleep lower baseline arousal.
Connect with peers. Talking with other adults who grew up with congenital heart disease, through groups such as the Canadian Congenital Heart Alliance or our Second Chapter community, reduces isolation.
If you have thoughts of self-harm, call or text 988 in Canada or the US.
ACHD special considerations
Some things always come first for congenital hearts. Many adults with a Fontan circulation or heart failure have a fluid limit. Many take anticoagulants, so any new supplement or medication, including for anxiety or sleep, needs a pharmacist check. Some anxiety medicines affect heart rhythm, so your ACHD team should know about anything prescribed elsewhere. Endocarditis prevention and pregnancy planning also need specialist input. Your ACHD team's guidance overrides any general advice, including ours.

The bottom line
If hospitals still make your heart race, years after your last childhood surgery, there's nothing wrong with you. Medical anxiety and trauma are common in adults with congenital heart disease, often untreated, and very treatable. Naming it, telling your ACHD team and getting the right support can make follow-up care feel possible again, and that protects your heart as well as your peace of mind.
Frequently asked questions
Can childhood heart surgery cause PTSD? It can. In one US clinic study, 11% of adults with congenital heart disease had elevated post-traumatic stress symptoms specifically related to their heart disease or treatment, and 21% had elevated symptoms overall. Many more have anxiety reactions to medical settings without meeting full PTSD criteria.
Is anxiety common in adults with congenital heart disease? Yes. In a Toronto adult congenital heart clinic, about half of patients met criteria for a mood or anxiety disorder, and most were not receiving treatment.
Why do I get so anxious before cardiology appointments? For people who spent much of childhood in hospital, medical settings can trigger learned stress responses. Anticipatory anxiety is common and treatable, and small changes such as bringing support and asking staff to explain each step can help.
Does medical anxiety affect follow-up care? It can lead to avoidance. About 42% of adults with congenital heart disease have had a gap in cardiology care of more than three years, and fear of medical settings is one reason some people stay away.
What treatments help medical anxiety and trauma? Cognitive behavioural therapy for anxiety, and trauma-focused therapies such as cognitive processing therapy, prolonged exposure or EMDR, are effective. Ask your ACHD team about psychological support or a referral.
