  • Why Fatigue in Adults With Congenital Heart Disease Is Different

"You look great." Adults with congenital heart disease hear it constantly, from friends, coworkers and sometimes doctors. Meanwhile, they're quietly arranging their week around how much energy they have left: no plans after work on Tuesdays, a nap before dinner, turning down the hike.

Fatigue is one of the most common complaints in adult congenital heart disease, and one of the least understood. It's also one that patients often stop mentioning, because they've had it so long it feels normal.

Here is the short answer. Fatigue in adults with congenital heart disease usually has several layers at once: a heart that pumps or fills differently, a heart rate that doesn't rise normally, lower oxygen in some conditions, iron deficiency, mood and sleep. Exercise capacity is often reduced even in people who feel well, so fatigue deserves a structured look rather than being accepted as normal.

It also shares a root with fatigue after a heart attack or stent: oxidative stress and chronic low-grade inflammation, which drain energy at the cellular level whatever the original diagnosis (see one inflammation, three hearts).

Is fatigue normal in adult congenital heart disease?

It's common, but that doesn't make it something to simply accept. In a landmark study, exercise capacity, measured as peak oxygen uptake during a cardiopulmonary exercise test, was reduced across every major type of adult congenital heart disease, including in many people who reported no symptoms at all. Lower exercise capacity also predicted a higher risk of hospitalisation or death (Diller et al., Circulation, 2005).

Two lessons follow. First, "I feel fine" and "my heart is working at full capacity" aren't the same thing; many people with congenital heart disease have adapted so gradually that they don't notice the gap. Second, a change in fatigue is information. If you're more tired than you were six months ago, that's worth reporting even if your last scan looked stable.

Why are adults with congenital heart disease so tired? Seven layers

1. How the heart pumps and fills. Leaking or narrowed valves, a weaker ventricle or an unusual circulation can limit how much blood the heart moves with each beat. In a Fontan circulation, there's no pumping chamber pushing blood through the lungs, so the body can't easily increase output during effort.

2. Heart rate that doesn't rise normally. Many adults with congenital heart disease have chronotropic incompetence: the heart rate doesn't climb as high as it should with exercise. Surgical scars affecting the heart's natural pacemaker, pacemaker settings and beta-blockers can all contribute.

3. Lower oxygen. In cyanotic conditions, oxygen levels are lower at baseline, so muscles and brain work with less fuel.

4. Iron deficiency. Iron is needed not just for haemoglobin but for energy production in muscle cells. People can be iron deficient with a normal haemoglobin, and in cyanotic patients the body needs even more iron. Ferritin and iron studies are worth asking about.

5. Mood and anxiety. In a Toronto adult congenital heart clinic, about half of patients met criteria for a mood or anxiety disorder, and most weren't receiving treatment (Kovacs et al., International Journal of Cardiology, 2009). Low mood and chronic worry are exhausting in their own right.

6. Sleep. Poor sleep, sleep apnoea and anxiety-driven insomnia all drain daytime energy.

7. Deconditioning and old advice. Many adults were told as children to avoid exertion. Years of limited activity reduce muscle fitness, which makes every task harder, a cycle that's often reversible with the right guidance.

On top of all seven sits the shared root. Chronic low-grade inflammation is linked to worse outcomes in congenital heart disease (Opotowsky et al., European Heart Journal, 2018) and is a recognised contributor to fatigue across many conditions (see ACHD and inflammation).

Which tests can help explain fatigue in ACHD?

Your ACHD team will decide what's appropriate, but these are reasonable questions to raise:

  • Cardiopulmonary exercise test (CPET): measures how well heart, lungs and muscles work together, and helps set a safe exercise prescription.

  • Imaging review: has anything changed in valves, conduits or ventricular function since your last scan?

  • Rhythm monitoring: a Holter or event monitor if you notice palpitations or sudden drops in energy.

  • Blood tests: haemoglobin, ferritin and iron studies, thyroid function, kidney function and, where relevant, BNP.

  • Mood screening: a short questionnaire such as the PHQ-9 or GAD-7.

  • Sleep assessment: if you snore, stop breathing at night or wake unrefreshed.

  • Medication review: especially beta-blockers and anything sedating.

What helps fatigue with a congenital heart

Once the layers are clearer, most people can do something about several of them at once:

  • An exercise prescription, not an exercise ban. Ask whether a CPET-guided plan is appropriate. Regular, well-matched activity improves muscle fitness and energy for most adults with congenital heart disease.

  • Correcting iron deficiency if tests show it, under your team's guidance.

  • Treating mood and anxiety. Psychological support is effective and often overlooked in congenital heart care.

  • Protecting sleep and getting sleep apnoea treated if present.

  • Pacing. Plan demanding tasks for your best hours and build rest in before you're depleted.

  • Working on the shared root with steady daily habits: movement, sleep, whole-food eating within any fluid or sodium limits, and stress care.

And if you've drifted out of specialist care, getting back in is the first step (see the ACHD transition gap).

  • 要的是运动处方,而不是运动禁令——询问是否适合以 CPET 指导的计划,规律且匹配的活动能改善大多数先心病成人的肌肉体能和精力;

  • 如检查显示缺铁,在团队指导下纠正;

  • 治疗情绪与焦虑——心理支持有效,却常在先心病照护中被忽视;

  • 保护睡眠,如有睡眠呼吸暂停应治疗;

  • 节奏管理——把费力的事安排在精力最好的时段,在耗尽前安排休息;

When fatigue needs prompt attention

Contact your ACHD team promptly if fatigue comes with new or worsening breathlessness, swelling in the legs or abdomen, rapid weight gain, palpitations, fainting or near-fainting, or bluer lips or fingers than usual. Call 911 for chest pain, severe breathlessness, fainting or any sign of stroke.

ACHD special considerations

Many adults with a Fontan circulation or heart failure have a fluid limit, so drinks and supplements must fit within it. Many take anticoagulants, so anything new, including iron or other supplements, needs a pharmacist check. Endocarditis prevention matters for many repaired hearts. Rhythm problems and cyanosis change what exercise and nutrition should look like. Your ACHD team's guidance overrides any general advice, including ours.

The bottom line

Fatigue with a congenital heart isn't laziness, and it isn't something you simply have to live with. It usually has several layers, from heart output and heart rate to iron, mood and sleep, and many of them can be measured and improved. Tell your ACHD team how your energy has changed, ask about the tests that fit, and give the shared root daily attention.

Frequently asked questions

Why am I always tired with congenital heart disease? Fatigue usually has several layers: how the heart pumps and fills, a heart rate that doesn't rise normally, lower oxygen in cyanotic conditions, iron deficiency, mood, sleep and deconditioning. Many of these can be measured and improved with your ACHD team.

Is fatigue normal after congenital heart surgery? It's common, but it shouldn't be accepted without a look. Exercise capacity is often reduced even in adults who feel well, and lower capacity has been linked to higher risk of hospitalisation, so a change in fatigue is worth reporting.

Why do people with a Fontan circulation get tired? In a Fontan circulation there's no pumping chamber pushing blood through the lungs, so the body can't easily increase heart output during effort. Heart rate response, oxygen levels, iron and fitness also play a part.

Can iron deficiency cause fatigue in adults with congenital heart disease? Yes. Iron is needed for muscle energy as well as haemoglobin, and people can be iron deficient with a normal haemoglobin. Ask about ferritin and iron studies, and only take iron under your team's guidance.

What tests can explain fatigue in ACHD? Useful options include a cardiopulmonary exercise test, imaging review, rhythm monitoring, blood tests including ferritin and thyroid function, mood screening, a sleep assessment and a medication review.
