  • Family Planning and Genetics: Will My Child Have a Heart Defect?

For many adults with congenital heart disease, the question arrives quietly, often in their late twenties or thirties: "If we have a baby, will they have what I have?" It can carry guilt, fear and a lot of uncertainty.

The answer is more reassuring than many people expect.

Here is the short answer. Most children of parents with congenital heart disease are born with healthy hearts. Congenital heart defects occur in about 1% of all births; when a parent has one, the risk for each child is typically a few percent, often higher when the mother is affected, and much higher if the defect is part of a genetic syndrome. Genetic counselling, preconception planning and fetal echocardiography help you plan.

Planning ahead also means looking after your own heart before and during pregnancy, including the shared root of oxidative stress and inflammation that congenital hearts share with heart attack survivors (see one inflammation, three hearts).

What is the chance my child will have a heart defect?

Background risk. Congenital heart defects affect about 1% of all births (CDC). Every pregnancy carries this baseline risk.

When a parent has congenital heart disease. In a large study of the children of adults with congenital heart disease, about 4% had a heart defect, and recurrence was generally higher when the mother was the affected parent (Burn et al., Lancet, 1998). Risk varies by the type of defect; some carry a higher recurrence than others. The child's defect, if present, isn't always the same as the parent's.

When a genetic syndrome is involved. Some congenital heart defects are part of a recognised genetic condition, such as 22q11.2 deletion syndrome, Noonan syndrome, Holt-Oram syndrome or Marfan syndrome. Many of these are inherited in an autosomal dominant pattern, meaning a child has up to a 50% chance of inheriting the condition, though how it affects the heart can vary widely (Pierpont et al., AHA Scientific Statement, Circulation, 2018).

These are general ranges. Your individual risk depends on your exact diagnosis, family history and any genetic findings, which is what genetic counselling is for.

Genetic counselling and testing: what to expect

A genetic counsellor or medical geneticist will review your diagnosis, family history and any features suggesting a syndrome. They may recommend testing, such as a chromosomal microarray or a targeted gene panel, to look for conditions like 22q11.2 deletion. Results can clarify your own health as well as your child's risk.

Testing is your choice, and counselling helps you decide what information is useful to you. In Canada, the Genetic Non-Discrimination Act means you can't be required to take a genetic test or share results to get insurance (see work, insurance and disclosure).

Fetal echocardiography and planning a pregnancy

If you or your partner has congenital heart disease, guidelines recommend a fetal echocardiogram, a detailed ultrasound of the baby's heart, usually around 18 to 22 weeks of pregnancy (Donofrio et al., Circulation, 2014). Many significant defects can be seen then, which gives time to plan delivery at a centre with paediatric cardiology if needed.

For people who will carry the pregnancy themselves, there's a second, equally important question: how will pregnancy affect my own heart? Pregnancy increases blood volume and heart workload considerably. Your ACHD team can estimate your risk class, review medications that aren't safe in pregnancy, such as some anticoagulants and blood pressure drugs, and plan monitoring. Ideally, this conversation happens before you start trying, along with standard steps such as folic acid.

Questions to bring to your teams

  • Is my heart defect part of a genetic syndrome, and should I be tested?

  • What's the recurrence risk for my specific diagnosis?

  • When should we have a fetal echocardiogram, and where?

  • What is my own risk during pregnancy, and which medicines need to change?

  • Where should I deliver, and who will be involved?

  • What follow-up will I need after the baby is born?

ACHD special considerations

If you're planning a pregnancy, check every medication and supplement with your ACHD team and pharmacist, including any nutrition plan such as Healos™; some are unsuitable in pregnancy and doses may need to change. Many adults with a Fontan circulation or heart failure have a fluid limit. Anticoagulants need a specific pregnancy plan. Endocarditis prevention matters around delivery for many repaired hearts. Rhythm problems, cyanosis and pulmonary hypertension can change pregnancy risk dramatically. Your ACHD team's guidance overrides any general advice, including ours.

How Healos™ supports adults with congenital heart disease

Planning a family is a long horizon, and so is congenital heart care. The 91-Day Protocol moves through Clear, Replenish and Repair, three sequenced stages of complementary cellular nutrition and lifestyle support, with check-ins at days 30, 60 and 91 that track energy, sleep and activity. Sovereign Monthly · Protect continues that support month to month. If you're planning or expecting a pregnancy, tell us and your ACHD team first; plans may need to pause or change.

The bottom line

Most children of parents with congenital heart disease have healthy hearts. The risk is a few percent for most people, higher when the mother is affected and much higher with some genetic syndromes. Genetic counselling clarifies your own numbers, a fetal echocardiogram around 18 to 22 weeks gives early answers, and a preconception visit with your ACHD team protects your own heart too.

Frequently asked questions

If I have congenital heart disease, will my child have it? Most children of affected parents have healthy hearts. Background risk is about 1%; with a parent who has congenital heart disease it's typically a few percent, about 4% in one large study, higher when the mother is affected and up to 50% with some genetic syndromes.

Is the risk higher if the mother has congenital heart disease? Generally yes. Studies have found higher recurrence in children of affected mothers than of affected fathers, though the exact risk depends on the specific defect.

Should I have genetic testing before pregnancy? It's worth discussing with a genetic counsellor, especially if your defect could be part of a syndrome such as 22q11.2 deletion or Noonan syndrome. Testing is your choice, and counselling helps you decide.

When is a fetal echocardiogram done? Usually around 18 to 22 weeks of pregnancy. It's recommended when a parent has congenital heart disease and can identify many significant defects in time to plan delivery.
