Lost Between Paediatric and Adult Cardiology: The ACHD Transition Gap
For many people born with a heart defect, the story goes like this. Surgery as a baby or toddler. Regular visits to a children's hospital, with a cardiologist who knew your name and your mother's phone number. Then, somewhere around 18, a letter about "transitioning to adult services". Maybe an appointment. Maybe not. Life gets busy: university, a first job, a move to a new city. You feel fine. Years go by.
If that sounds familiar, you're far from alone. And it's one of the most common, and most fixable, risks in adult congenital heart disease.
Here is the short answer. Many adults with congenital heart disease drift out of cardiology care in their late teens and twenties, often after leaving paediatric services. In one large US study, about four in ten had a gap of more than three years. Gaps matter because problems such as valve leaks and rhythm disorders can progress silently, and reconnecting with an adult congenital heart clinic is the first step back.
There's a second thread worth knowing from the start. Whatever your original repair, the long-term biology you're now managing, oxidative stress and chronic low-grade inflammation, is the same root that heart attack and stent patients work on (see one inflammation, three hearts). Getting back into specialist care and supporting that shared root go hand in hand.
How common is it for adults with congenital heart disease to be lost to follow-up?
More common than most people expect. In the HEART-ACHD study, which looked at adults attending congenital heart clinics in the US, about 42% had experienced a gap in cardiology care of more than three years. The first gap most often began around age 19 to 20, right at the point of leaving paediatric care (Gurvitz et al., JACC, 2013).
That matters because adults are now the majority of people living with congenital heart disease. In Quebec, adults made up about two-thirds of everyone with the condition by 2010 (Marelli et al., Circulation, 2014). A generation of survivors that medicine worked hard to save is now responsible for managing its own follow-up, often without anyone explaining why it still matters.
Why do young adults with congenital heart disease drift out of care?
Almost nobody decides to stop. The drift has ordinary causes:
"I was told it was fixed." Many people heard as children that surgery had corrected their heart, and reasonably concluded they were done.
Feeling well. Most long-term problems develop slowly and silently, so there's no symptom to prompt a visit.
The handover itself. Paediatric care is built around parents. Adult care assumes you'll book, attend and follow up on your own, often at a different hospital.
Life transitions. Moving for school or work, changing provinces or countries, and losing access to the clinic that knew you.
Insurance and work. Coverage changes, time off for appointments and uncertainty about disclosure all add friction.
Anxiety. For people who spent childhood in hospitals, staying away can feel like a relief. That's understandable, and it's worth naming.
None of these is a personal failing. They're the predictable result of a system that hands responsibility to young adults at the busiest moment of their lives.
「医生说修好了」——很多人小时候听说手术已经纠正了心脏,自然以为结束了;
感觉良好——多数长期问题缓慢而无声地发展,没有症状提醒就医;
Why gaps in congenital heart care matter
A repaired heart is usually not a "normal" heart. Many repairs leave changes that can evolve over decades:
Leaking or narrowing valves, such as pulmonary valve regurgitation after tetralogy of Fallot repair, which can gradually enlarge the right ventricle.
Rhythm disorders, because scar tissue from surgery can create electrical short circuits.
Changes in heart pumping function, sometimes without symptoms until late.
Liver and circulation effects in people with a Fontan circulation.
Acquired risks such as high blood pressure, cholesterol and weight gain, which add to the original condition as people age.
Many of these are very treatable if found early, and much harder if found late. In one study, adults with congenital heart disease who had a lapse in care were about three times more likely to need urgent cardiac intervention (Yeung et al., International Journal of Cardiology, 2008). And when Quebec increased referrals to specialised ACHD centres, that was associated with lower mortality (Mylotte et al., Circulation, 2014).
瓣膜反流或狭窄(例如法洛四联症修复后的肺动脉瓣反流,会逐渐使右心室扩大);
心律问题(手术瘢痕可能造成电路「短路」);
心脏泵血功能变化(有时到晚期才出现症状);
Fontan 循环患者的肝脏与循环影响;
ACHD specialist clinic or general cardiologist: who should follow you?
US guidelines recommend that adults with moderate or complex congenital heart disease be followed in collaboration with an ACHD specialist, and that even people with simpler lesions have at least an initial ACHD assessment (Stout et al., Circulation, 2019). Canadian guidance follows the same principle.
ACHD specialists are cardiologists with additional training in congenital anatomy and its long-term consequences. They know, for example, what a 30-year-old Fontan circulation needs, or when a leaking pulmonary valve should be replaced. A general cardiologist can be an excellent partner, especially for routine issues and acquired risks, but the specialist review sets the plan.
If you're not sure which category your heart falls into, that question alone is a good reason to book an assessment.
How to get back into congenital heart care: a four-step roadmap
Step 1: Gather your history. Ask the children's hospital, or your parents, for your diagnosis, operation names and dates, and the most recent echocardiogram or MRI report. Even a one-page summary helps enormously.
Step 2: Get a referral. Ask your family doctor or walk-in clinic for a referral to an adult congenital heart disease clinic. In Canada, the Canadian Congenital Heart Alliance (CCHA) and, in the US, the Adult Congenital Heart Association (ACHA) list specialist centres.
Step 3: Prepare for the first visit. Bring your history, a full list of medications and supplements, and your questions. Write down any symptoms, even ones you've dismissed: breathlessness on stairs, palpitations, swelling, fatigue.
Step 4: Build a rhythm. Ask how often you should be seen and which tests you'll need, then put the next appointment in your calendar before you leave.
Alongside that roadmap, it helps to work on the shared root every day: steady movement, sleep, a whole-food eating pattern, mood and medication consistency. That's where structured support between visits makes the biggest difference.
ACHD special considerations
Some things are different for congenital hearts and always come first. Many adults with a Fontan circulation or heart failure have a fluid limit, so drinks and supplements must fit within it. Many take anticoagulants, so anything new needs a pharmacist check. Endocarditis prevention, including good dental care and knowing when antibiotics are recommended, matters for many repaired hearts. Rhythm problems and cyanosis change what exercise and nutrition should look like. And pregnancy planning needs a specialist conversation well in advance. Your ACHD team's guidance overrides any general advice, including ours.

The bottom line
Outgrowing the children's hospital doesn't mean outgrowing care. If you've drifted, you're in very common company, and getting back is simpler than it feels: gather your history, get a referral, prepare one good first visit and build a rhythm. Then give the years between appointments the same attention as the appointments themselves.
Frequently asked questions
Do adults with congenital heart disease still need a cardiologist? Yes, in most cases. Guidelines recommend that adults with moderate or complex congenital heart disease be followed with an ACHD specialist, and that people with simpler lesions have at least an initial specialist assessment, because problems such as valve leaks and rhythm disorders can develop silently over decades.
What happens if I stopped seeing my heart doctor after childhood? You're not alone: about 42% of adults in one large US study had a gap of more than three years. The best step is to gather your history and ask your family doctor for a referral to an adult congenital heart disease clinic. Gaps have been linked to a higher chance of needing urgent intervention later.
How do I find an adult congenital heart disease clinic? Ask your family doctor for a referral. In Canada, the Canadian Congenital Heart Alliance lists resources and specialist centres; in the US, the Adult Congenital Heart Association has a clinic directory.
I was told my heart was fixed as a child. Do I still need check-ups? Usually yes. Most repairs leave changes, such as valve leaks or scar tissue, that can evolve over decades. Many are very treatable when found early, which is why regular specialist follow-up matters.
Can Healos™ replace my ACHD clinic? No. Healos™ provides complementary nutritional and lifestyle support between visits, starting with the diagnosis and medication list you share with us. Your ACHD team's guidance always comes first.
