Growing Older With Congenital Heart Disease: The First Generation of Adult Survivors
When many of today's adults with congenital heart disease were born, nobody could promise their parents a long life. Surgeons were inventing operations as they went, and "adulthood" was the hopeful goal. That goal has largely been met. Now a new question has arrived, one nobody had a playbook for: what does it look like to grow old with a congenital heart?
Here is the short answer. Most children born with heart defects now reach adulthood, and the first large generation of survivors is reaching midlife. With age come new priorities: heart failure becomes the leading cause of death in adult congenital heart disease, arrhythmias and valve reoperations become more common, and acquired risks such as blood pressure and cholesterol add up. Lifelong specialist follow-up and daily work on the shared root help you stay ahead.
That shared root, oxidative stress and chronic low-grade inflammation, is the same one heart attack survivors work on (see one inflammation, three hearts), and it matters more with every decade.
The first generation to grow old with a congenital heart
Surgical success has changed the shape of congenital heart disease. In Quebec, adults made up about two-thirds of everyone living with the condition by 2010 (Marelli et al., Circulation, 2014), and that proportion keeps growing. People who had early repairs of tetralogy of Fallot, transposition or a Fontan circulation in the 1970s and 1980s are now in their forties and fifties.
This is genuinely new territory. Much of what's known about long-term outcomes is being learned in real time from this generation, which is one reason staying connected with an ACHD clinic matters so much: you benefit from what's learned, and you contribute to it (see the ACHD transition gap).
What changes with age in adult congenital heart disease
Heart failure. In a large contemporary cohort of adults with congenital heart disease, chronic heart failure was the leading cause of death (Diller et al., Circulation, 2015). Ventricles that have worked under unusual pressure or volume for decades can gradually weaken.
Arrhythmias. Surgical scars and stretched chambers make atrial flutter and other rhythm problems more common with each decade.
Valves and conduits. Many repairs have a lifespan; replacement or reintervention, often now by catheter, becomes part of midlife care.
Acquired risks. High blood pressure, diabetes, weight gain and high cholesterol arrive just as they do for everyone else, and coronary disease becomes a real possibility (Lui et al., AHA Scientific Statement, Circulation, 2017).
Other organs. Kidney and liver function, particularly with a Fontan circulation, need monitoring.
Life stages. Menopause, changes in exercise tolerance and, eventually, frailty all interact with a congenital heart.
How to stay ahead as you get older
Keep your ACHD follow-up rhythm, including imaging and rhythm checks at the intervals your team recommends. About 42% of adults with congenital heart disease have had a gap of more than three years (Gurvitz et al., JACC, 2013); midlife is the worst time for one.
Know your acquired-risk numbers: blood pressure, LDL cholesterol, blood sugar and weight, checked like any adult's.
Watch for early heart failure signs: new breathlessness, swelling, rapid weight gain, needing more pillows at night or a drop in exercise tolerance. Report changes early.
Stay active with a plan suited to your heart (see exercise with congenital heart disease).
Plan ahead: discuss future procedures, pregnancy or menopause, travel and insurance with your team before they become urgent.
Work on the shared root daily: sleep, stress, whole-food eating within any fluid or sodium limits, and not smoking.
ACHD special considerations
Many adults with a Fontan circulation or heart failure have a fluid limit. Many take anticoagulants, and the number of medications tends to grow with age, so a yearly pharmacist review is worthwhile. Endocarditis prevention remains important, especially after valve or conduit replacement. Rhythm problems and cyanosis shape what exercise and nutrition should look like. Your ACHD team's guidance overrides any general advice, including ours.
How Healos™ supports adults with congenital heart disease
Healos™ is built for the long stretches between appointments, which get longer and more important with age. The 91-Day Protocol moves through Clear, Replenish and Repair, three sequenced stages of complementary cellular nutrition and lifestyle support, with check-ins at days 30, 60 and 91 that track energy, weight, activity and sleep, the early signals worth bringing to your ACHD team. Sovereign Monthly · Protect continues that support month to month for long-term maintenance and can be cancelled any time.
The bottom line
You're part of a generation nobody fully planned for, and that's a remarkable thing. Growing older with a congenital heart brings new priorities: heart failure, rhythm, valves and the same acquired risks everyone faces. Stay connected with your ACHD clinic, know your numbers, report early warning signs and give the shared root daily attention. The plan for growing old is yours to write.
Frequently asked questions
Can people with congenital heart disease live into old age? Most children born with heart defects now reach adulthood, and many adults are living into midlife and beyond. Long-term outcomes depend on the type of defect, repairs and ongoing care, which is why lifelong ACHD follow-up matters.
What changes as adults with congenital heart disease get older? Heart failure, arrhythmias and the need for valve or conduit reinterventions become more common, and acquired risks such as high blood pressure, diabetes and high cholesterol add to the original condition.
Can adults with congenital heart disease develop heart failure? Yes. In a large contemporary cohort, chronic heart failure was the leading cause of death in adults with congenital heart disease, so early signs such as new breathlessness, swelling or rapid weight gain should be reported promptly.
Do adults with congenital heart disease need cholesterol and blood pressure checks? Yes. As they age, adults with congenital heart disease face the same acquired risks as everyone else, so blood pressure, cholesterol, blood sugar and weight should be checked regularly.
