  • Depression After Heart Surgery, Heart Attack and Stroke: Different Numbers, Same Mechanism

Families often describe the change before the patient does. "He survived the bypass, but he's not himself." "She's lost interest in everything since the stroke." "He says he's fine, but he hasn't left the house in weeks."

Here is the short answer. Depression is common after every kind of heart and brain event: about one in five heart attack survivors meets criteria for major depression, and nearly one in three stroke survivors experiences depression. The numbers differ, but the mechanism overlaps: stress hormones, inflammation, disrupted sleep and the loss of a familiar life. Depression slows recovery, and it's treatable.

How common is depression after a heart attack, heart surgery or stroke?

After a heart attack: roughly one in five survivors meets criteria for major depression when assessed by structured interview, and many more have milder symptoms (Thombs et al., Journal of General Internal Medicine, 2006).

After heart surgery: depression is common before and after bypass and valve surgery, and in congenital heart disease mood and anxiety problems are frequent and often untreated, sometimes rooted in years of childhood hospital care.

After a stroke: about 31% of survivors experience depression (Hackett & Pickles, International Journal of Stroke, 2014). Stroke can affect brain circuits involved in mood directly, on top of the emotional impact of sudden disability.

The common thread: these numbers are well above the general population, and depression is frequently missed because its symptoms, fatigue, poor sleep and low energy, overlap with recovery itself.

Same mechanism: why the heart and brain affect mood

Different events, overlapping biology:

Stress hormones. A life-threatening event activates the body's stress system, the hypothalamic-pituitary-adrenal (HPA) axis. Persistently high cortisol is linked to low mood, poor sleep and higher blood pressure.

Inflammation. Heart attacks, surgery and stroke all trigger inflammation, and inflammatory signals act on the brain to produce fatigue, withdrawal and low mood. The same shared root of oxidative stress and inflammation that slows physical recovery also weighs on mood (see one inflammation, three hearts).

Autonomic balance. Depression is linked to reduced heart rate variability, a marker of a nervous system stuck in "alert" mode.

Sleep. Pain, medications, sleep apnoea and worry fragment sleep, and poor sleep both causes and worsens depression.

Loss. Roles, independence, work, driving, the sense of a body you could trust. Grief for those losses is real, and it can slide into depression.

  • 应激激素——危及生命的事件激活身体的应激系统(HPA 轴),皮质醇持续偏高与情绪低落、睡眠差和血压升高相关;

  • 炎症——心梗、手术和卒中都会引发炎症,炎症信号作用于大脑,产生疲劳、退缩和低落,拖慢身体康复的那个氧化应激与炎症共同根源同样压在情绪上;

  • 自主神经平衡——抑郁与心率变异性降低相关,提示神经系统卡在「警戒」状态;

  • 睡眠——疼痛、药物、睡眠呼吸暂停和担忧打碎睡眠,而睡眠差既导致也加重抑郁;

Why depression matters for physical recovery

Depression isn't only about how someone feels. The American Heart Association formally recognises it as a risk factor for poorer outcomes after an acute coronary syndrome (Lichtman et al., Circulation, 2014). Part of the reason is practical: depression makes it harder to take medications, attend rehab, exercise, eat well and sleep. Part is biological, through the stress and inflammation pathways above.

In the Healos™ framework, that's why mood acts as a multiplier on every other part of recovery: a perfect plan followed by someone in untreated depression delivers only a fraction of its potential. Treating depression isn't separate from treating the heart or brain; it's part of it.

Depression after heart attack, surgery and stroke: what's different

The core is shared, but each group has its own features, summarised in the table below. After a heart attack, fear of recurrence and trauma symptoms often travel with depression (see heart attack PTSD). After stroke, depression can be harder to recognise when speech or thinking is affected, and it overlaps heavily with post-stroke fatigue (see post-stroke fatigue). In congenital heart disease, anxiety rooted in childhood medical experiences is common and often untreated.

What helps: treatments that work after heart and brain events

Depression after a cardiac event or stroke responds to treatment, and several approaches have been tested specifically in these patients:

  • Screening. A short questionnaire such as the PHQ-9 takes a few minutes. Ask for it, or ask your family to help you raise it.

  • Talking therapies. Cognitive behavioural therapy and other structured psychological treatments are effective for depression and can be adapted for cardiac and stroke patients.

  • Collaborative care. In the Bypassing the Blues trial, telephone-delivered collaborative care after bypass surgery improved mood and quality of life (Rollman et al., JAMA, 2009).

  • Medication, when appropriate. In the SADHART trial, the antidepressant sertraline was safe in people with a recent heart attack or unstable angina and major depression (Glassman et al., JAMA, 2002). Your doctor will choose a medicine that suits your heart rhythm, other medications and bleeding risk. Never start or stop an antidepressant on your own.

  • Exercise and cardiac or stroke rehab. Regular activity improves mood as well as fitness.

  • Sleep, daylight and connection. Treating sleep apnoea, getting morning light and staying connected with people all help.

If you or someone you love has thoughts of self-harm, call or text 988 in Canada or the US, or go to the nearest emergency department.

How families can help

Families are often the first to notice and the best placed to help. Name what you're seeing kindly and specifically ("you haven't wanted to see anyone since the stroke"). Offer to go to the next appointment and raise it together. Encourage small, regular activity rather than pushing big goals. And look after yourself too: caregivers of heart and stroke survivors are at high risk of burnout and depression themselves.

How Healos™ supports recovery

Healos doesn't provide therapy or prescribe medication. It gives recovery a steady structure that includes mood. The 91-Day Protocol moves through Clear, Replenish and Repair, providing complementary cellular nutrition and lifestyle support, with check-ins at days 30, 60 and 91 that track mood and sleep alongside activity and medications, so changes are visible to you and your care team. Sovereign Monthly · Recovery offers the same support month to month.

The bottom line

After a heart attack, heart surgery or stroke, depression is common, it shares a biology across all three, and it slows recovery. It's also very treatable. Ask for screening, take low mood as seriously as chest pain or weakness, and let recovery include the mind as well as the body.

Frequently asked questions

Is depression common after a heart attack? Yes. Roughly one in five heart attack survivors meets criteria for major depression, and many more have milder symptoms. It's often missed because fatigue and poor sleep overlap with recovery.
