Post-Stroke Fatigue: Why Sleeping All Day Doesn't Fix It
Families often describe it the same way. "Dad sleeps half the day and still says he's exhausted." "She managed a short visit with the grandchildren and then needed the rest of the afternoon in bed." "He's not lazy. He was never like this."
The person living with it usually puts it more simply: "It's not like being tired. It's like the battery just stops."
Here is the short answer. About half of stroke survivors experience post-stroke fatigue, a deep, persistent exhaustion that rest doesn't relieve. It's thought to reflect changes in the brain itself, often made worse by sleep apnoea, depression, deconditioning and medications. It can last months or longer, but pacing, graded activity, treating sleep and mood problems, and reviewing medications can make it more manageable.
This guide explains what's happening, how to tell fatigue from depression, and the strategies that actually help.
What is post-stroke fatigue, and how common is it?
Post-stroke fatigue is a lasting sense of exhaustion, physical, mental or both, that is out of proportion to activity and doesn't fully recover with rest. A systematic review found that about half of stroke survivors report it (Cumming et al., International Journal of Stroke, 2016).
It can appear even after an excellent physical recovery, which is part of what makes it so confusing for families. Someone who walks well, speaks clearly and looks "back to normal" may still run out of energy after an hour of conversation, a trip to the supermarket or a morning of light work.
It's also more than a feeling. Fatigue after stroke is linked to lower participation in rehabilitation, more difficulty returning to work and lower quality of life, which is why it deserves the same attention as weakness or speech problems.
Why are stroke survivors so tired? The main causes
Post-stroke fatigue rarely has a single cause. Several factors usually overlap:
The brain is working harder. The brain is about 2% of body weight but uses about 20% of the body's energy (Raichle & Gusnard, PNAS, 2002). After a stroke, tasks that used to be automatic, such as walking, following a conversation or filtering background noise, take more effort and more energy.
Inflammation and recovery biology. A stroke triggers inflammation in and around the brain, and the repair process continues for weeks to months. That shared root of inflammation and oxidative stress links stroke recovery with heart recovery (see one inflammation, three hearts).
Sleep apnoea. Sleep-disordered breathing is extremely common after stroke: in one meta-analysis, about 72% of stroke patients had some degree of it, and about 38% had moderate or worse sleep apnoea (Johnson & Johnson, Journal of Clinical Sleep Medicine, 2010). Fragmented, low-oxygen sleep leaves people exhausted however long they stay in bed.
Depression. About 31% of stroke survivors experience depression (Hackett & Pickles, International Journal of Stroke, 2014), and depression and fatigue feed each other.
Deconditioning. Time in hospital and reduced activity cost muscle strength and stamina quickly, especially in older adults.
Medications. Some blood pressure medicines (including beta-blockers), sedatives, certain antidepressants and anti-seizure drugs can add to fatigue. Never stop a medication on your own, but it's worth asking your doctor or pharmacist to review the list.
Other medical causes. Anaemia, thyroid problems, low vitamin B12, infections and blood sugar problems can all cause or worsen fatigue and are simple to check.
大脑在更努力地工作——大脑约占体重 2% 却消耗约 20% 的能量,卒中后走路、跟上对话、过滤背景噪音等原本自动完成的事情都需要更多努力和能量;
炎症与修复生物学——卒中会在大脑内外引发炎症,修复过程持续数周到数月,这一炎症与氧化应激的共同根源也把卒中康复和心脏康复联系在一起;
睡眠呼吸暂停——卒中后极为常见,一项荟萃分析中约 72% 的卒中患者有一定程度的睡眠呼吸障碍,约 38% 为中度以上,碎片化、低氧的睡眠让人躺多久都累;
抑郁——约 31% 的卒中幸存者经历抑郁,抑郁与疲劳相互加重;
体能下降——住院和活动减少会迅速损失肌力和耐力,老年人尤甚;
药物——部分降压药(包括 β 阻滞剂)、镇静剂、某些抗抑郁药和抗癫痫药可能加重疲劳,切勿自行停药,但值得请医生或药剂师复查用药;
Fatigue, tiredness or depression? How to tell the difference
These three overlap, and they often coexist, but the differences matter because they point to different solutions. Ordinary tiredness follows effort and improves with a good night's sleep. Post-stroke fatigue arrives out of proportion to effort and doesn't fully recover with rest. Depression is marked by low mood, loss of interest or pleasure and feelings of hopelessness, alongside low energy.
A useful question: "If you had the energy, would you want to do it?" People with fatigue usually say yes; they want to, they just can't. People with depression more often say they've lost interest. Either way, if low mood or loss of interest lasts more than two weeks, tell the care team. Depression after stroke is common and treatable.
How long does fatigue last after a stroke?
For some people, fatigue eases over the first few months as the brain recovers and fitness returns. For others it lasts a year or longer, and it can appear for the first time months after the stroke. There's no reliable way to predict who will recover quickly.
That uncertainty is frustrating, but it also means fatigue is worth actively managing rather than waiting out. Survivors who learn to work with their energy, rather than against it, usually report more good days, even when the underlying fatigue hasn't fully gone.
What helps post-stroke fatigue: practical strategies
Research on treatments for post-stroke fatigue is still developing; a 2020 systematic review found the evidence for any single intervention remains limited (Su et al., Journal of Clinical Medicine, 2020). In practice, a combination of approaches tends to help most:
Pace with an energy budget. Think of each day as having a limited number of energy units. Plan the important things for the time of day you have most energy, break tasks into smaller pieces and schedule rest before you're exhausted, not after.
Build activity gradually. Too little activity worsens deconditioning; too much triggers crashes. Programmes that combine graded activity with cognitive strategies have reduced fatigue in trials (Zedlitz et al., Stroke, 2012). Your rehab team can help set the starting level.
Get checked for sleep apnoea. Snoring, pauses in breathing, morning headaches or waking unrefreshed are all reasons to ask for a sleep assessment. Treatment can make a large difference.
Treat low mood. Depression is common, treatable and often a hidden driver of fatigue.
Review medications and blood tests. Ask about medicines that may add to fatigue, and about checking blood count, thyroid, B12 and blood sugar.
Protect the basics. Regular sleep and wake times, daylight in the morning, enough fluids within any limits your team sets, and regular meals with enough protein all support the brain's energy supply.
Explain it to family. Fatigue is invisible. Families who understand it as a real neurological symptom, not laziness, make pacing far easier.
用「精力预算」管理节奏——把每天看作只有有限的精力单位,把重要的事安排在精力最好的时段,把任务拆小,在累垮之前而不是之后休息;
循序渐进地活动——活动太少会加重体能下降,太多会导致崩溃,结合渐进活动与认知策略的项目在试验中减轻了疲劳,康复团队可帮助设定起点;
检查睡眠呼吸暂停——打鼾、呼吸暂停、晨起头痛或醒来不解乏,都是要求睡眠评估的理由,治疗可能带来很大改变;
治疗低落情绪——抑郁常见、可治疗,且常是隐藏的疲劳推手;
复查药物和血液检查——询问可能加重疲劳的药物,并检查血常规、甲状腺、B12 和血糖;
守住基础——规律作息、早晨日光、在医疗团队设定的范围内充足饮水、规律进餐并摄入足够蛋白质,都支持大脑的能量供应;
When fatigue needs urgent attention
Gradual, persistent fatigue is common after stroke. Sudden new exhaustion is different, especially if it comes with any sign of stroke: loss of Balance, Eye or vision changes, Face drooping, Arm weakness, Speech difficulty. Remember BE FAST and call 911 immediately. Also contact your care team promptly for fatigue with chest pain, breathlessness, fever, confusion or a marked change from usual. If low mood includes thoughts of self-harm, call or text 988 in Canada or the US.

The bottom line
Post-stroke fatigue is real, common and different from ordinary tiredness. It's driven by the brain's extra workload and recovery biology, and often amplified by sleep apnoea, depression, deconditioning and medications, several of which are very treatable. Rest alone rarely fixes it. Pacing, gradual activity, checking sleep and mood, and reviewing medications usually make the days more manageable, and give recovery room to happen.
