Heart Attack PTSD: When "I Thought I Was Going to Die" Doesn't Go Away
Many heart attack survivors can describe the moment in detail years later: the pressure in the chest, the sirens, the face of the paramedic, the thought, clear as anything, "I'm going to die." For most people, that memory slowly loses its sharpness. For some, it doesn't.
They relive it at night. They avoid the stairs, the gym, the restaurant where it happened. They check their pulse a dozen times a day and feel every flutter as a warning.
Here is the short answer. About one in eight heart attack survivors develops significant post-traumatic stress symptoms: intrusive memories, avoidance and constant alertness to every heartbeat. It isn't weakness, and it matters for the heart, because it's linked to roughly double the risk of another cardiac event or death. Trauma-focused therapies help, and the first step is telling your care team.
Can a heart attack cause PTSD?
Yes. Post-traumatic stress disorder isn't limited to combat or accidents. Any experience in which you believe your life is in danger can trigger it, and a heart attack fits that description exactly.
A meta-analysis of 24 studies found that about 12% of patients develop clinically significant PTSD symptoms after an acute coronary syndrome (Edmondson et al., PLoS One, 2012). Many more have milder symptoms that still shape daily life.
What makes heart-related trauma unusual is that the threat isn't in the past. Someone who survived a car crash can avoid that road. A heart attack survivor carries the source of the danger inside their chest, and every heartbeat can feel like a reminder. That's why cardiac PTSD often looks less like flashbacks and more like constant vigilance.
Signs of PTSD after a heart attack
Post-traumatic stress usually shows up in three clusters:
Re-experiencing. Intrusive memories, nightmares, or a surge of panic triggered by something that recalls the event: a siren, a hospital smell, a similar chest sensation.
Avoidance. Steering clear of places, activities or conversations linked to the heart attack. In cardiac patients this often means avoiding exercise, skipping rehab or even avoiding medications, because taking them is a daily reminder.
Hyperarousal. Feeling constantly on guard: checking the pulse, scanning the body for symptoms, poor sleep, irritability and difficulty concentrating.
Many people also notice a sense of a "foreshortened future", a feeling that planning ahead is pointless. If several of these have lasted more than a month and are affecting your life, that's worth raising with your doctor.
Why heart attack PTSD matters for your heart
This isn't only about how you feel. In the Edmondson meta-analysis, PTSD symptoms after an acute coronary syndrome were associated with roughly double the risk of recurrent cardiac events or death (PLoS One, 2012).
Part of the explanation is behavioural. In one study of heart attack survivors, post-traumatic stress symptoms were linked to poorer adherence to aspirin and a higher risk of adverse outcomes (Shemesh et al., Psychosomatic Medicine, 2004). Avoiding exercise, rehab and medications quietly removes the very things that protect the heart.
Part is biological. Chronic stress keeps stress hormones and inflammatory signalling elevated and disrupts sleep, all of which feed the same shared root of oxidative stress and inflammation that drives heart disease. In the Healos framework, that's why mind-body-spirit state acts as a multiplier on every other part of recovery: unresolved fear can quietly cancel out good nutrition and good intentions.
Normal fear or PTSD? And the one rule about chest symptoms
Some fear after a heart attack is normal and even protective; it's what makes people take their medications and show up to rehab. The difference is intensity and duration: normal fear gradually eases and doesn't run your life, while PTSD persists, spreads and starts to dictate what you can and can't do (see fear after heart surgery is not the same as anxiety).
One rule matters more than any other: never decide on your own that chest pain is "just anxiety". If you have chest pain or pressure, breathlessness, pain spreading to the arm, jaw or back, or fainting, call 911. Many survivors find it reassuring to agree a clear plan with their cardiologist in advance: which sensations mean call 911, which mean call the clinic, and which can be noted for the next visit. A written plan reduces both danger and fear.
What helps heart attack PTSD
PTSD is one of the most treatable mental health conditions, and cardiac PTSD responds to the same approaches.
Trauma-focused therapy. Clinical guidelines recommend trauma-focused psychotherapies such as cognitive processing therapy, prolonged exposure and trauma-focused cognitive behavioural therapy for PTSD (American Psychological Association, 2017). EMDR is also widely used. These therapies help the memory become a memory again, rather than a constant alarm.
Cardiac rehab, with support. Rehab is a safe, supervised place to relearn that your heart can handle effort. Tell the team if fear is making it hard to attend; many programmes have psychologists or can refer you.
Screening for depression. PTSD and depression often travel together; roughly one in five heart attack survivors meets criteria for major depression (Thombs et al., Journal of General Internal Medicine, 2006).
Sleep and routine. Regular sleep and wake times, daylight and gentle daily movement lower baseline arousal.
Medication review. If fear is making you skip medications, say so. Your team can help, and some people find that reframing medication as protection rather than a reminder of illness makes a real difference.
Peer support. Talking with people who've been through the same thing reduces isolation. Our Second Chapter community is one place to start.

How Healos™ supports recovery after a heart attack
Healos™ doesn't provide psychotherapy, and it never replaces trauma-focused treatment. What it does is give recovery a steady structure, which matters a great deal when fear makes everything feel uncertain.
The 91-Day Protocol moves through Clear, Replenish and Repair, three sequenced stages of complementary cellular nutrition and lifestyle support, with check-ins at days 30, 60 and 91 that track mood and sleep alongside activity and medications, so changes are visible to you and your cardiologist. Sovereign Monthly · Recovery offers the same support month to month. Both run alongside your cardiac and mental health care.
The bottom line
Surviving a heart attack can leave a mark that doesn't show on any scan. If the fear hasn't eased, if you're reliving it, avoiding things or checking your pulse all day, you're not being weak, and you're far from alone. Heart attack PTSD is common, it matters for your heart, and it's very treatable. Tell your care team, keep a clear plan for chest symptoms and let recovery include your mind as well as your arteries.
Frequently asked questions
Can a heart attack cause PTSD? Yes. About 12% of patients develop clinically significant post-traumatic stress symptoms after an acute coronary syndrome. Any experience in which you believe your life is in danger can trigger PTSD, and a heart attack fits that description.
Is it normal to be afraid of another heart attack? Some fear is normal and can even help you stay on track with medications and rehab. If fear persists for more than a month, keeps growing or stops you from doing everyday things, it's worth raising with your doctor.
Does PTSD after a heart attack affect my heart? It can. PTSD symptoms after an acute coronary syndrome have been associated with about double the risk of recurrent cardiac events or death, partly through avoidance of exercise and medications and partly through stress and sleep disruption.
How is heart attack PTSD treated? Trauma-focused psychotherapies such as cognitive processing therapy, prolonged exposure and trauma-focused cognitive behavioural therapy are recommended, and EMDR is widely used. Cardiac rehab, depression screening, good sleep and peer support also help.
How do I know if chest pain is anxiety or my heart? Don't try to decide on your own. Chest pain or pressure, breathlessness, pain spreading to the arm, jaw or back, or fainting means call 911. Agree a written plan with your cardiologist in advance about which symptoms need emergency care.
