Sauna vs Cold Plunge: What Each Does to Your Heart
Sauna vs Cold Plunge: What Each Does to Your Heart
Heat and cold have become wellness trends: infrared saunas in gyms, ice baths on social media, cold plunges in backyards. Both promise better circulation, less stress and a healthier heart. For someone thinking about long-term heart health, or living with heart disease, the details matter.
Here is the short answer. Regular sauna use has the stronger heart evidence: in a Finnish study of 2,315 men, those using a sauna four to seven times a week had about half the risk of fatal cardiovascular disease compared with once a week, though the data are observational. Cold plunges have much thinner evidence and trigger a cold-shock response that can be risky for people with heart disease. Anyone with a heart condition should check with their team first.
What a sauna does to the heart
In a traditional Finnish sauna at around 80 °C, skin temperature rises quickly, blood vessels widen, heart rate climbs to levels similar to light or moderate exercise and blood pressure often dips slightly during and after. Repeated over years, that pattern may improve blood vessel function.
The best-known evidence comes from Finland. In the Kuopio Ischaemic Heart Disease Risk Factor Study, 2,315 middle-aged men were followed for about 20 years. Compared with men who used a sauna once a week, those who used one four to seven times a week had about 50% lower risk of fatal cardiovascular disease (Laukkanen et al., JAMA Internal Medicine, 2015).
It's an impressive association, but it's observational. Men who sauna often in Finland may also be healthier, more relaxed or more socially connected, and the study included only men. Sauna looks like a pleasant, probably beneficial habit for healthy adults; it isn't a treatment.
What a cold plunge does to the heart
Sudden immersion in cold water triggers the cold shock response: an involuntary gasp, rapid breathing and sharp rises in heart rate and blood pressure within the first minute. At the same time, if the face is immersed or breath is held, the dive reflex pushes heart rate down. Physiologists describe this simultaneous push-pull on the heart as autonomic conflict, and it may provoke arrhythmias in susceptible people (Shattock & Tipton, Journal of Physiology, 2012).
The claimed benefits, such as better mood, recovery or metabolism, rest on far thinner evidence. A systematic review found possible effects on stress hormones and insulin sensitivity, but most studies were small and of limited quality (Espeland et al., International Journal of Circumpolar Health, 2022). There's no long-term outcome data showing cold plunges protect the heart.
If you have heart disease: who should be careful
Sauna: people with stable coronary disease, controlled blood pressure or stable heart failure can often use a sauna in moderation, but ask your cardiologist first. Avoid it after a recent heart attack or procedure until cleared, and with unstable chest pain, severe aortic stenosis, uncontrolled blood pressure or recent fainting. Be cautious with diuretics, vasodilators or a fluid limit, as heat lowers blood pressure and increases fluid loss. Never combine sauna with alcohol.
Cold plunge: most cardiologists would advise people with coronary disease, arrhythmias, heart failure, a dilated aorta, congenital heart disease or uncontrolled blood pressure to avoid sudden cold immersion unless specifically cleared. If you're cleared, cool showers or gradual entry are far gentler than plunging in.
Practical tips if you're cleared
Sauna: start with 5–10 minutes at a moderate temperature, sit rather than lie on the top bench, drink water before and after (within any fluid limit), cool down gradually and get out if you feel dizzy, unwell or have chest symptoms.
Cold: start with a cool shower for the last 30 seconds, never plunge alone, avoid putting your face under, keep sessions short and skip it entirely if you feel unwell.
Neither replaces the basics: blood pressure, cholesterol, daily movement, sleep and not smoking do far more for your heart than any temperature trend (see heart disease starts decades before symptoms).

The bottom line
Sauna has decades of observational data linking frequent use to lower cardiovascular death in healthy Finnish men; cold plunges have thin evidence and a real cold-shock risk. For healthy adults, a sauna habit is a reasonable, enjoyable addition. For anyone with heart disease, both need a conversation with your cardiologist first, and cold immersion is usually one to skip.
Frequently asked questions
Is sauna good for your heart? In a Finnish study of 2,315 men, using a sauna four to seven times a week was associated with about 50% lower risk of fatal cardiovascular disease compared with once a week. The data are observational, so sauna is best seen as a probably beneficial habit, not a treatment.
Is a cold plunge safe for your heart? For healthy people it's usually tolerated, but sudden cold immersion triggers a cold shock response with sharp rises in heart rate and blood pressure that may provoke arrhythmias. People with heart disease should generally avoid it unless cleared.
Can I use a sauna after a heart attack? Many people with stable heart disease can use a sauna in moderation once cleared by their cardiologist. Avoid it after a recent heart attack or procedure, with unstable symptoms, severe aortic stenosis or uncontrolled blood pressure, and never with alcohol.
Sauna or cold plunge: which is better for heart health? Sauna has much stronger, though observational, evidence for heart health. Cold plunges have thin evidence and higher acute risk. Neither replaces blood pressure and cholesterol control, activity, sleep and not smoking.
