Heart Disease Starts Decades Before Symptoms: What a Normal Check-Up Can Miss
Most people picture heart disease as something that arrives suddenly: a crushing chest pain, an ambulance, a stent. And most people who have a heart attack in their fifties or sixties would tell you they felt fine the week before.
They probably did. That's exactly the problem.
Here is the short answer. Heart disease usually starts decades before the first symptom. Fatty streaks appear in the arteries in many teenagers, and in one study 63% of symptom-free adults aged 40 to 54 already had plaque. A routine check-up can miss this because standard tests don't measure plaque or inherited risk, so knowing which extra tests to ask about, and supporting your heart now, matters.
The same biology connects prevention and recovery: oxidative stress and chronic low-grade inflammation drive plaque long before a heart attack, and they're what heart attack and stent patients keep working on afterwards (see one inflammation, three hearts). The difference is that you get to start earlier.
Can you have heart disease and feel completely fine?
Yes, and most people with early heart disease do. The evidence spans seven decades.
In 1953, pathologists examining young American soldiers killed in Korea, with an average age of 22, found visible coronary atherosclerosis in 77% of them (Enos et al., JAMA, 1953). The later PDAY study confirmed that fatty streaks in the arteries are common by the teens and twenties.
Modern imaging tells the same story in midlife. In the Spanish PESA study, researchers scanned 4,184 office workers aged 40 to 54 who had no heart symptoms at all. 63% already had plaque in at least one artery (Fernández-Friera et al., Circulation, 2015).
And sometimes the first symptom is the last. About half of men and nearly two-thirds of women who die suddenly of coronary heart disease had no previous symptoms (AHA Heart Disease and Stroke Statistics). Arteries can also widen outward to accommodate plaque, so blood flow, and therefore how you feel, stays normal for years (see the 10-year cellular timeline).
Why a normal check-up can miss early heart disease
A routine check-up is designed to catch common, measurable risk factors, and it does that reasonably well. But it has blind spots:
It doesn't look at plaque. Standard blood tests and a stethoscope can't see what's in your artery walls.
A standard cholesterol panel misses inherited risk. About one in five people has an elevated lipoprotein(a), a largely genetic particle that raises risk even when LDL looks fine (Kronenberg et al., European Heart Journal, 2022). It isn't part of the routine panel.
One blood pressure reading isn't enough. A single clinic measurement can be falsely reassuring or falsely alarming; home readings over several days tell you more.
Risk calculators average you out. Tools that estimate 10-year risk are useful, but they can underestimate risk in people with a strong family history, inflammatory conditions or certain ancestries, including South Asian heritage.
Lifestyle and mood don't show up on a lab report. Sleep, chronic stress, activity and diet shape risk for years before any number changes.
"Normal" on a routine report usually means "no red flags today". It doesn't mean "nothing is happening".
它不看斑块——标准血液检查和听诊器看不到动脉壁里的情况;
标准血脂检查漏掉遗传风险——约五分之一的人脂蛋白(a)偏高,这种主要由遗传决定的颗粒即使 LDL 看起来正常也会增加风险,而它不在常规检查中;
一次血压读数不够——单次诊室测量可能误导,连续几天的家庭测量更有信息量;
风险计算器会「平均化」你——对有强家族史、炎症性疾病或某些族裔(包括南亚裔)的人可能低估风险;
Which heart tests are worth asking your doctor about?
None of these replaces your doctor's judgement, and not everyone needs all of them. But they're reasonable questions to bring to your next visit, especially if you have a family history or other risk factors:
Lipoprotein(a): Canadian guidelines recommend measuring it once in adulthood (Pearson et al., Canadian Journal of Cardiology, 2021). It rarely changes, so one test answers the question for life.
ApoB or non-HDL cholesterol: a more complete picture of the particles that build plaque.
HbA1c: catches prediabetes, which raises heart risk years before diabetes is diagnosed.
Home blood pressure: a week of correctly taken readings before your appointment.
Coronary artery calcium (CAC) score: a low-dose CT scan that detects calcified plaque. It's most useful for people at intermediate risk, where the result could change treatment decisions; it isn't a routine test for everyone.
hs-CRP: a marker of low-grade inflammation that can add context in some people.
Bring your family history too. A parent or sibling with heart disease before 55 (men) or 65 (women) is one of the most important facts your doctor can know.
脂蛋白(a)——加拿大指南建议成年后测一次,它很少变化,一次检测终身有效;
ApoB 或非 HDL 胆固醇——更完整地反映形成斑块的颗粒;
HbA1c——发现糖尿病前期,它在确诊糖尿病前多年就增加心脏风险;
家庭血压——就诊前一周正确测量的读数;冠脉钙化积分(CAC)——低剂量 CT 检测钙化斑块,对中等风险、结果可能改变治疗决策的人最有用,并非人人需要的常规检查;
Family history: the risk factor you can't change, but can act on
If a parent or sibling had a heart attack, stent or bypass young, defined as before 55 in men or 65 in women, your own risk is meaningfully higher. That's partly genes, including inherited traits such as high lipoprotein(a) or familial high cholesterol, and partly shared habits: how a family eats, moves, sleeps and handles stress.
The useful way to think about family history isn't as a verdict but as an early warning that buys you time. It's a reason to ask for the right tests earlier, to know your numbers better than your parents knew theirs, and to start working on the shared root while your arteries still have decades of flexibility.
It's also worth telling relatives what you find. If your lipoprotein(a) turns out to be high, your siblings and children may carry it too, and a single blood test can tell them.
What you can do now, decades before symptoms
The encouraging side of a decades-long process is that you have decades to influence it. The same foundations that protect heart attack survivors work even better earlier:
Know your numbers and track them over time, not just once.
Move every day; consistency beats intensity.
Protect your sleep and manage chronic stress, both closely tied to inflammation.
Eat a pattern built on vegetables, fruit, whole grains, legumes, nuts and oily fish.
Don't smoke, and keep alcohol modest.
If your doctor recommends medication such as a statin or blood pressure treatment, take it consistently. Lifestyle and medication work together; neither replaces the other.
In the Healos framework, these factors add up and your mind-body-spirit state multiplies them. Long-term heart health is less about a single dramatic change and more about the slow, steady pressure you apply to the shared root of oxidative stress and inflammation.

The bottom line
Feeling fine and having healthy arteries aren't the same thing. Heart disease builds quietly for decades, routine check-ups have blind spots, and a handful of extra questions, about lipoprotein(a), ApoB, home blood pressure and, for some people, a calcium score, can fill them. The earlier you start working on the shared root, the more time you have to change the story.
Frequently asked questions
Can you have heart disease and feel completely fine? Yes. In the PESA study, 63% of symptom-free adults aged 40 to 54 already had plaque, and about half of men and nearly two-thirds of women who die suddenly of coronary heart disease had no previous symptoms.
Can a normal check-up miss heart disease? It can. Routine check-ups don't image plaque, standard cholesterol panels don't include lipoprotein(a), and a single blood pressure reading can be misleading. A normal result usually means no red flags today, not that nothing is developing.
What heart tests should I ask my doctor about? Reasonable questions include a one-time lipoprotein(a) test, ApoB or non-HDL cholesterol, HbA1c, a week of home blood pressure readings and, for people at intermediate risk, a coronary artery calcium score. Your doctor will decide what's appropriate.
Should I get a coronary calcium scan? A coronary artery calcium score is most useful for people at intermediate risk, where the result could change decisions such as starting a statin. It isn't a routine test for everyone; discuss it with your doctor.
What is lipoprotein(a)? Lipoprotein(a) is a largely inherited cholesterol-carrying particle that raises heart and stroke risk even when LDL is normal. About one in five people has an elevated level, and Canadian guidelines recommend testing it once in adulthood.
