Overwork and Heart Risk in Your 50s and 60s | Healos™
Overwork and the Heart: Why Busy Professionals in Their 50s and 60s Are at Risk
Every autumn and winter, the same kind of news seems to arrive: a respected professor, physician or business leader in their fifties or sixties, at the height of their career, collapses at a conference, on a business trip or at their desk. Colleagues ask the same question each time: they seemed so healthy, how could this happen?
The answer is usually that the risk was building quietly for years, in exactly the people least likely to slow down and check.
Here is the short answer. Busy professionals in their 50s and 60s often look healthy while risk quietly builds. Working 55 or more hours a week is linked to about 35% higher stroke risk and 17% higher risk of dying from heart disease, job strain adds risk of its own, and heart attacks are more common in winter. A check-up this fall, attention to warning signs and protected sleep are the most useful steps.
Long hours and job strain: what the evidence shows
In the first global analysis of its kind, the World Health Organization and the International Labour Organization estimated that working 55 or more hours a week is associated with about 35% higher risk of stroke and 17% higher risk of dying from ischaemic heart disease, compared with a standard 35 to 40 hours. They estimated that long working hours led to about 745,000 deaths from stroke and heart disease worldwide in 2016 (WHO/ILO, Environment International, 2021).
It isn't only the hours. Job strain, high demands with little control, was associated with about 23% higher risk of coronary heart disease in a meta-analysis of nearly 200,000 workers (Kivimäki et al., Lancet, 2012).
The likely pathways are familiar: chronically raised stress hormones and blood pressure, short and broken sleep, irregular meals, little exercise, more alcohol and, crucially, skipped check-ups. All of them feed the shared root of inflammation and oxidative stress behind heart disease (see one inflammation, three hearts).
Why the 50s and 60s, and why now
Age and accumulated risk. By the 50s, decades of blood pressure, cholesterol and stress have had time to build plaque, often silently (see heart disease starts decades before symptoms). A family history makes it more likely (see your parent had an early heart attack).
The season. Heart attacks are about 50% more frequent in winter than in summer (Spencer et al., JACC, 1998). Cold raises blood pressure and makes blood more likely to clot, and winter brings respiratory infections, heavier meals and less activity. October and November are the right time to check, before the peak.
The busiest people check least. People who look after everyone else, and travel constantly, are often the last to book their own check-up or to take a symptom seriously.
Don't explain away the warning signs
Sudden doesn't always mean without warning. In one large study, about half of people who had a sudden cardiac arrest had warning symptoms in the four weeks before, most often chest pain or breathlessness (Marijon et al., Annals of Internal Medicine, 2016). Busy people are especially good at explaining them away: indigestion, jet lag, too much coffee, a stressful week.
Take these seriously, especially if they're new or happen with effort: chest pain, pressure or tightness; unusual breathlessness; pain spreading to the arm, jaw, neck or back; palpitations with dizziness; fainting or near-fainting; or unusual exhaustion. Call 911, or the local emergency number when travelling, for severe or persistent symptoms. Don't drive yourself, and don't wait until the meeting is over.
A fall checklist for busy professionals
Modern medicine is at its best when it comes to finding problems and saving lives in an emergency. But most heart disease risk builds quietly over the ten or twenty years before the first symptom. That window depends on timely screening, healthy habits kept up over time, and the right preventive care.
Book the check-up you've been postponing: blood pressure, LDL cholesterol, a one-time lipoprotein(a), HbA1c and waist size; a calcium score if your doctor thinks it would change decisions (see preventing heart disease after 50).
Guard sleep: seven to nine hours, and ask about sleep apnoea if you snore (see sleep debt and heart risk).
Cap the week: aim to keep regular weeks below 55 hours and protect at least one day without work.
Move daily, even 20 to 30 minutes of brisk walking between meetings.
On the road: take medicines on schedule across time zones, carry a list of them, keep alcohol modest, and know the local emergency number.
Get your flu shot and keep vaccinations up to date.
Tell someone close to you about any new symptom, and let them push you to act.
血压、LDL、一次脂蛋白(a)、HbA1c 和腰围,医生认为有用时做钙化积分;
守护睡眠:7–9 小时,打鼾要问睡眠呼吸暂停;
给每周设上限:常规周尽量低于 55 小时,至少保留一天不工作;
每天活动,哪怕会议间快走 20–30 分钟;
出差时跨时区按时服药、随身带药单、适度饮酒、知道当地急救电话;
接种流感疫苗;

The bottom line
Looking healthy and being healthy aren't the same, especially after 50 with a demanding job. Long hours, job strain, short sleep, travel and winter all add up, and the people carrying the most responsibility often check the least. This fall, book the check-up, take warning signs seriously and protect your sleep. The people who rely on you need you for the long run.
Frequently asked questions
Can overwork cause a heart attack? Long working hours and job strain are linked to higher risk. WHO and ILO estimated that working 55 or more hours a week is associated with about 35% higher stroke risk and 17% higher risk of dying from ischaemic heart disease, likely through stress, blood pressure, poor sleep and missed check-ups.
Why do people who seem healthy die suddenly of heart attacks? Plaque can build silently for decades, and about half of people who have a sudden cardiac arrest had warning symptoms in the previous four weeks that were often explained away. Feeling fine isn't the same as having healthy arteries.
What heart checks should I get in my 50s? Blood pressure (including home readings), LDL cholesterol, a one-time lipoprotein(a), HbA1c and waist size, plus a calcium score if your doctor thinks it would change decisions. Start earlier if a parent had early heart disease.
How can I protect my heart when travelling for work? Take medicines on schedule across time zones, carry a medication list, sleep as much as you can, keep alcohol modest, stay active and know the local emergency number. Don't ignore chest pain or breathlessness because of a meeting.
