Sleep Debt and Heart Risk: How Many Hours Actually Matter
Ask a busy professional how they're sleeping and you'll often hear a number with an apology attached: "Five hours, but I'm used to it." Sleep is usually the first thing traded away for work, family and screens, and the last thing people associate with their heart.
The research says it belongs near the top of the list.
Here is the short answer. For heart health, most adults need about seven to nine hours of sleep a night, and the American Heart Association now counts sleep as one of its eight core measures of cardiovascular health. Regularly sleeping five or six hours or less is linked to about 48% higher risk of coronary heart disease, and an irregular sleep schedule may matter as much as total hours.
How much sleep do you need for heart health?
In 2022, the American Heart Association updated its framework for cardiovascular health, now called Life's Essential 8, and added sleep alongside diet, activity, nicotine, weight, cholesterol, blood sugar and blood pressure. For adults, the optimal range is 7 to 9 hours a night (Lloyd-Jones et al., Circulation, 2022).
The evidence behind it is substantial. A meta-analysis of prospective studies found that short sleep, typically five or six hours or less, was associated with a 48% higher risk of developing or dying from coronary heart disease and a 15% higher risk of stroke (Cappuccio et al., European Heart Journal, 2011). Interestingly, regularly sleeping much longer than nine hours was also linked to higher risk, though long sleep can be a sign of other health problems rather than a cause.
These are associations from observational studies, not proof that sleep alone causes heart disease. But the biology is consistent, and sleep is one of the most modifiable factors on the list.
Why poor sleep strains the heart
During healthy sleep, blood pressure and heart rate fall, a pattern called nocturnal dipping, giving the cardiovascular system its daily recovery window. Short or broken sleep cuts that window short. It also:
Raises blood pressure, both at night and the next day.
Keeps stress hormones elevated and tilts the nervous system toward "alert".
Increases inflammation and oxidative stress, the shared root behind heart disease (see one inflammation, three hearts).
Disrupts appetite hormones and blood sugar control, nudging weight and insulin resistance up.
Drains the energy and motivation needed to exercise and cook well the next day.
升高夜间和次日血压;
让应激激素保持偏高、神经系统偏向「警戒」;
增加炎症和氧化应激——心脏病背后的共同根源;
扰乱食欲激素和血糖控制,推高体重和胰岛素抵抗;
Does sleep regularity matter as much as hours?
Possibly more. Using wrist-worn activity data from UK Biobank participants, researchers found that sleep regularity, going to bed and waking at consistent times, was a stronger predictor of mortality risk than sleep duration (Windred et al., Sleep, 2024).
That has practical implications. Sleeping five hours on weeknights and ten on weekends gives a respectable weekly average but a very irregular rhythm, sometimes called social jet lag. Weekend catch-up sleep can ease some of the next-day effects, but it doesn't fully undo a week of short, irregular nights. A consistent wake-up time, even on weekends, is one of the simplest changes with the biggest payoff.
When it isn't just sleep debt: sleep apnoea and insomnia
If you sleep seven or eight hours and still wake exhausted, the problem may be sleep quality rather than quantity. Loud snoring, gasping, morning headaches and daytime sleepiness suggest sleep apnoea, which is common, often undiagnosed and linked to high blood pressure and atrial fibrillation (see sleep apnoea after a heart attack or stroke). Persistent difficulty falling or staying asleep for three months or more may be chronic insomnia, which responds well to cognitive behavioural therapy for insomnia (CBT-I). Both are worth raising with your doctor.
A seven-night sleep reset
You don't need perfect sleep. You need a rhythm your heart can count on. Try this for one week:
Fix your wake-up time, seven days a week, and count back seven and a half hours for a target bedtime.
Get daylight within an hour of waking, ideally outdoors, even on grey Canadian mornings.
Stop caffeine by early afternoon and keep alcohol modest; it fragments the second half of the night.
Dim screens and lights an hour before bed.
Keep the bedroom cool, dark and quiet.
If you can't sleep after about 20 minutes, get up and do something calm in low light until you feel sleepy.
Track bedtime, wake time and how rested you feel each morning. After a week, the pattern usually tells you what to change next.
固定起床时间,一周七天都一样,倒推七个半小时作为目标上床时间;
醒来一小时内接触日光,最好在户外,即使是灰蒙蒙的加拿大早晨;
下午早些时候就停止咖啡因,适度饮酒(酒精会打碎后半夜的睡眠);
睡前一小时调暗屏幕和灯光;卧室保持凉爽、黑暗、安静;

The bottom line
Your heart keeps score of your sleep. Aim for seven to nine hours, protect a consistent wake-up time and treat loud snoring or chronic insomnia as health issues rather than personality traits. Of all the measures in Life's Essential 8, sleep may be the one you can start improving tonight.
Frequently asked questions
How many hours of sleep do I need for heart health? Most adults need about 7 to 9 hours a night. The American Heart Association includes sleep in Life's Essential 8, its core measures of cardiovascular health, with 7 to 9 hours as the optimal range.
Does lack of sleep cause heart disease? Short sleep is strongly associated with heart disease: in a large meta-analysis, it was linked to 48% higher risk of coronary heart disease and 15% higher risk of stroke. These are associations, but the biological pathways, including blood pressure, stress hormones and inflammation, are consistent.
Is it bad to sleep in on weekends to catch up? Catch-up sleep can ease some next-day effects, but big swings between weeknights and weekends create an irregular rhythm. In UK Biobank data, sleep regularity predicted mortality risk more strongly than duration, so a consistent wake-up time helps most.
Can poor sleep raise blood pressure? Yes. Short or broken sleep reduces the normal night-time fall in blood pressure and can raise it the next day. Sleep apnoea is also strongly linked to high blood pressure.
