Your Parent Had an Early Heart Attack: What Family History Means for You
Many people carry a date in the back of their mind: the age their father or mother had a heart attack. As it gets closer, the question gets louder. Am I next?
Here is the short answer. If a parent or sibling had a heart attack or other heart disease early, before 55 in men or 65 in women, your own risk is roughly doubled. Some of that comes from inherited conditions such as familial hypercholesterolaemia or high lipoprotein(a), which simple blood tests can find. Genes aren't destiny: in people with high genetic risk, a healthy lifestyle was linked to about 46% lower risk of coronary events.
Family history isn't a verdict. It's an early warning that gives you time to act, as long as you use it.
How much does a parent's early heart attack raise your risk?
Doctors usually count family history as premature when a first-degree relative, a parent, brother or sister, had a heart attack, stent, bypass, stroke or sudden cardiac death before 55 (men) or 65 (women).
In the long-running Framingham Offspring Study, premature heart disease in a parent was associated with about double the risk of cardiovascular disease in their children, even after accounting for cholesterol, blood pressure, smoking and diabetes (Lloyd-Jones et al., JAMA, 2004). Risk is higher still when more than one relative is affected, or when they were especially young.
A parent with high blood pressure, high cholesterol or diabetes, even without an early heart attack, also raises your chance of developing the same conditions, through both genes and shared habits.
Inherited causes worth testing for
Part of family risk comes from specific, findable conditions:
Familial hypercholesterolaemia (FH). An inherited condition causing very high LDL from birth. It affects roughly 1 in 250 people and is often undiagnosed (Nordestgaard et al., European Heart Journal, 2013). Clues include LDL around 4.9 mmol/L (190 mg/dL) or higher, early heart disease in relatives and, sometimes, fatty deposits on tendons or around the eyes. Treatment works well, and finding one person with FH often leads to diagnosing several relatives, called cascade screening.
High lipoprotein(a). Lp(a) is a cholesterol-carrying particle largely set by genes and not shown on a standard cholesterol panel. Canadian guidelines recommend measuring it once in adulthood (Pearson et al., Canadian Journal of Cardiology, 2021). A high result doesn't change overnight, but it changes how intensively your doctor treats other risks.
Inherited tendencies toward high blood pressure and diabetes, which respond to early detection and treatment.
Your family-history checklist: what to check and when
Bring your family history to your doctor, as specifically as you can: who, what happened, and at what age. Then ask about:
Blood pressure, including a week of home readings.
LDL cholesterol (or ApoB) and lipoprotein(a) once.
HbA1c or fasting glucose.
Waist size and weight.
A coronary artery calcium score, if you're at intermediate risk and the result would change decisions.
Earlier and more frequent checks than the general population, often starting in your 20s or 30s if FH is suspected or a parent had heart disease very young.
If you're found to have FH or very high Lp(a), tell your siblings and children, who may benefit from testing too.
Modern medicine is at its best when it comes to finding problems and saving lives in an emergency. But most heart disease risk builds quietly over the ten or twenty years before the first symptom. That window depends on timely screening, healthy habits kept up over time and the right preventive care (see preventing heart disease after 50).
把家族史尽量具体地告诉医生:谁、发生了什么、几岁时;
然后询问:血压,包括一周家庭读数;LDL(或 ApoB)以及一次脂蛋白(a);
HbA1c 或空腹血糖;腰围和体重;
中等风险、结果可能改变决策时询问冠脉钙化积分;
Can lifestyle offset genetic heart risk?
Yes, substantially. In a study of more than 55,000 people, those at high genetic risk who followed a favourable lifestyle, not smoking, no obesity, regular physical activity and a healthy diet, had about 46% lower relative risk of coronary events than those with an unfavourable lifestyle (Khera et al., NEJM, 2016).
The same daily foundations that matter after a heart attack work on the shared root of inflammation and oxidative stress well before symptoms appear (see one inflammation, three hearts). For people with FH or very high LDL, lifestyle alone isn't enough, and medication is usually needed; the two work together.

The bottom line
A parent's early heart attack roughly doubles your risk, and part of it may come from findable conditions such as familial hypercholesterolaemia or high Lp(a). Tell your doctor exactly who was affected and when, ask for the right tests earlier than usual, share results with relatives, and keep the daily foundations going. Genes load the dice; what you do next still decides a great deal.
Frequently asked questions
My father had a heart attack at 50. What does that mean for me? A heart attack in a parent before 55 (men) or 65 (women) counts as premature family history and roughly doubles your own risk. Tell your doctor, check blood pressure, LDL, a one-time lipoprotein(a) and HbA1c, and start earlier than usual.
Is heart disease hereditary? Partly. Genes influence cholesterol, blood pressure, blood sugar and lipoprotein(a), and some conditions such as familial hypercholesterolaemia are directly inherited. Shared family habits also play a role.
What is familial hypercholesterolaemia? An inherited condition that causes very high LDL from birth, affecting roughly 1 in 250 people. It's very treatable, and diagnosing one person often leads to testing and treating relatives.
Can a healthy lifestyle offset genetic heart risk? To a large extent. Among people at high genetic risk, a favourable lifestyle was associated with about 46% lower relative risk of coronary events. People with very high LDL usually also need medication.
