Sleep Apnoea After a Heart Attack or Stroke: Why Screening Matters
Often it's the partner who notices first. "He snores like a freight train, then goes quiet, and then gasps." "She's exhausted all day no matter how early she goes to bed." After a heart attack or stroke, those observations deserve more attention than they usually get.
Here is the short answer. Sleep apnoea, repeated pauses in breathing during sleep, is very common after a heart attack or stroke and often goes undiagnosed. In stroke patients, about 72% have some sleep-disordered breathing and 38% have moderate or worse sleep apnoea. A simple questionnaire and a home sleep test can identify it, and treatment improves sleepiness, mood and quality of life.
What is sleep apnoea, and why does it matter for the heart and brain?
In obstructive sleep apnoea, the airway collapses repeatedly during sleep. Each pause drops blood oxygen, jolts the nervous system awake and spikes blood pressure and heart rate, sometimes dozens of times an hour. In central sleep apnoea, more common in heart failure and after some strokes, the brain intermittently stops signalling the body to breathe.
Night after night, that pattern adds stress to the cardiovascular system. Sleep apnoea is linked to high blood pressure that's hard to control, atrial fibrillation, poorer sleep quality and daytime fatigue. It also feeds the shared root of oxidative stress and inflammation that heart and stroke recovery depend on calming (see one inflammation, three hearts).
How common is sleep apnoea after a heart attack or stroke?
Very. In a meta-analysis of stroke and TIA patients, about 72% had some degree of sleep-disordered breathing and about 38% had moderate or worse sleep apnoea (Johnson & Johnson, Journal of Clinical Sleep Medicine, 2010). Rates are also high in people with coronary artery disease, heart failure and atrial fibrillation.
Yet most cases are never diagnosed. In the general population, an estimated 80% or more of people with moderate to severe obstructive sleep apnoea don't know they have it (Young et al., Sleep, 1997). After a cardiac event or stroke, symptoms like fatigue and poor concentration are easily blamed on recovery itself (see post-stroke fatigue).
Signs of sleep apnoea: a simple screening checklist
The STOP-Bang questionnaire, developed in Toronto, is a widely used eight-question screen (Chung et al., Anesthesiology, 2008). Count one point for each "yes":
Snoring loudly
Tired, sleepy or fatigued during the day
Observed pauses in breathing, choking or gasping during sleep
Pressure: high blood pressure, or being treated for it
BMI over 35
Age over 50
Neck circumference over 40 cm
Gender: male
A higher score means a higher likelihood of obstructive sleep apnoea, and many heart and stroke patients will score at least three just from age and blood pressure. That's not a diagnosis, but it's a good reason to ask your doctor about a sleep test. Other clues include morning headaches, waking to urinate several times a night and low mood.
How is sleep apnoea diagnosed?
Diagnosis is simpler than many people expect. For most adults with suspected obstructive sleep apnoea, a home sleep apnoea test, a small device worn overnight in your own bed, is enough. Some people, including those with heart failure, significant lung disease or suspected central sleep apnoea, may need an overnight study in a sleep lab. Your family doctor, cardiologist or neurologist can refer you; ask whether it's covered in your province or by your plan.
What treatment can, and can't, do
The main treatment for obstructive sleep apnoea is CPAP, a mask that keeps the airway open with gentle air pressure. Alternatives include oral appliances, positional therapy and weight loss where relevant.
It's worth being honest about the evidence. In the SAVE trial of 2,717 people with cardiovascular disease and sleep apnoea, CPAP did not reduce heart attacks, strokes or cardiovascular deaths overall, although many participants used it for only a few hours a night. It did improve snoring, daytime sleepiness, mood and quality of life (McEvoy et al., NEJM, 2016).
So the strongest reason to get tested is how you feel and function: better sleep, more energy, steadier mood and often easier blood pressure control. For people recovering from a heart attack or stroke, those gains make every other part of recovery, from exercise to medication routines, easier to sustain.
How Healos™ supports recovery
Healos™ plans don't diagnose or treat sleep apnoea. They track it as part of the bigger picture. The 91-Day Protocol moves through Clear, Replenish and Repair, providing complementary cellular nutrition and lifestyle support, with check-ins at days 30, 60 and 91 that include sleep and daytime energy, so patterns that suggest sleep apnoea are visible and can be raised with your doctor. Sovereign Monthly · Recovery offers the same support month to month.
The bottom line
After a heart attack or stroke, sleep apnoea is common, often missed and easy to screen for. If you snore, stop breathing at night, wake unrefreshed or feel sleepy all day, take the STOP-Bang checklist to your doctor and ask about a sleep test. Treatment may not be a cure for heart disease, but better sleep makes recovery much easier to live.
Frequently asked questions
Is sleep apnoea common after a stroke? Yes. In a meta-analysis, about 72% of stroke and TIA patients had some degree of sleep-disordered breathing and about 38% had moderate or worse sleep apnoea.
Should I be tested for sleep apnoea after a heart attack? If you snore, have observed breathing pauses, wake unrefreshed or feel sleepy during the day, ask your doctor. A STOP-Bang score of three or more is a common reason to consider testing, often with a home sleep test.
Does CPAP lower the risk of another heart attack or stroke? Large trials such as SAVE did not show a reduction in cardiovascular events overall, but CPAP improved snoring, daytime sleepiness, mood and quality of life, which helps the rest of recovery.
What are the signs of sleep apnoea? Loud snoring, observed pauses in breathing or gasping, daytime sleepiness, morning headaches, waking to urinate at night and high blood pressure are common signs.
