  • Mental Health Apps vs Therapy After a Cardiac Event

After a heart attack or stroke, a lot of people quietly download a meditation or mood app. It feels like a sensible, private first step, and for some it helps. For others, months go by with an app they rarely open while anxiety or low mood settles in.

Here is the short answer. Apps and online programs can help with mild anxiety or low mood after a heart attack or stroke, especially guided programs with a human coach. But evidence for unguided apps is mixed, a cardiac-specific online CBT trial showed little effect with low uptake, and many apps share data. For moderate or severe symptoms, trauma symptoms or any thoughts of self-harm, therapy with a professional is the right level of care.

Why mental health support matters after a cardiac event

Roughly one in five heart attack survivors meets criteria for major depression (Thombs et al., Journal of General Internal Medicine, 2006), and depression and anxiety make it harder to take medications, exercise and attend rehab. Getting the right support is part of physical recovery, not separate from it (see depression after heart and brain events).

The question isn't really "app or therapy?" It's "what level of support fits how I'm doing right now?"

What the evidence says about apps and online programs

Guided beats unguided. In a large individual patient data meta-analysis, internet-based cognitive behavioural therapy for depression worked better when it included guidance from a therapist or coach than when people used it alone (Karyotaki et al., JAMA Psychiatry, 2021).

Cardiac patients are a special case. In the Swedish U-CARE Heart trial, an internet-based CBT program for heart attack survivors with depression or anxiety did not significantly improve symptoms compared with usual care, and many participants used little of it (Norlund et al., Journal of Medical Internet Research, 2018). Fatigue, competing appointments and the emotional weight of a heart event may make self-directed programs harder to stick with.

Privacy matters. In an analysis of top-ranked depression and smoking-cessation apps, most transmitted user data to third parties such as Facebook or Google (Huckvale et al., JAMA Network Open, 2019). Check an app's privacy policy, and prefer tools offered through your health system or a recognised provider.

Apps still have a place: breathing exercises, sleep routines and mood tracking are useful, low-risk supports, especially alongside other care.

Matching support to how you're doing: a stepped approach

Many health systems use stepped care: the lightest effective support first, stepping up when needed. A simple guide, with your PHQ-9 score as one input (see depression screening after a heart attack):

  • Mild symptoms (PHQ-9 about 5–9): self-help tools, apps with good privacy, cardiac rehab, exercise, sleep and social connection. Recheck in a few weeks.

  • Moderate symptoms (about 10–14) or symptoms lasting more than a few weeks: guided online CBT or therapy, plus a conversation with your doctor. In Ontario, for example, publicly funded structured psychotherapy programs offer CBT at no cost.

  • Moderately severe or severe symptoms (15+), trauma symptoms such as flashbacks or avoidance, or depression affecting medications or daily life: therapy with a qualified professional, and a medical review of options including medication (see heart attack PTSD).

  • Any thoughts of self-harm: call or text 988 now, or go to the nearest emergency department.

  • 轻度症状(PHQ-9 约 5–9)——自助工具、隐私良好的 App、心脏康复、运动、睡眠和社交联结,几周后复查;

  • 中度症状(约 10–14)或持续数周以上——有指导的在线认知行为疗法或心理治疗,并与医生沟通,例如安大略省有公费的结构化心理治疗项目免费提供认知行为疗法;

  • 中重度或重度(15 分以上)、闪回或回避等创伤症状、或抑郁已影响服药和日常生活——由合格专业人员提供心理治疗,并进行包括药物在内的医学评估;

    任何自伤念头——立即拨打或发短信至 988,或前往最近的急诊。

Questions to ask before choosing

  • Is this app or program evidence-based, and is it guided by a person?

  • Who can see my data, and is it shared with advertisers?

  • Does the therapist understand heart or stroke recovery, or can they coordinate with my care team?

  • Is there a cost, and is it covered by my province or benefits plan?

  • Does my cardiac or stroke rehab program offer psychological support?

How Healos™ supports recovery

Healos™ doesn't provide therapy and never replaces it. In the Healos framework, mind-body-spirit state is the multiplier on every other part of recovery, so mood is tracked rather than left to chance. The 91-Day Protocol moves through Clear, Replenish and Repair, providing complementary cellular nutrition and lifestyle support, with check-ins at days 30, 60 and 91 that include mood and sleep, so changes reach your care team early. Sovereign Monthly · Recovery offers the same support month to month.

The bottom line

Apps can be a helpful first step for mild symptoms, especially guided programs with good privacy. But after a heart attack or stroke, self-directed tools often aren't enough on their own, and moderate or severe symptoms, trauma or any thoughts of self-harm need professional care. Match the support to how you're really doing, and step up when you need to.

Frequently asked questions

Can a mental health app help after a heart attack? For mild anxiety or low mood, apps and especially guided online programs can help, alongside rehab, exercise and sleep. A cardiac-specific online CBT trial showed little effect with low uptake, so self-directed tools often aren't enough on their own.

Is online therapy as good as in-person therapy? Guided online CBT can work well for many people with depression or anxiety, and it's more effective than unguided programs. Video therapy with a qualified professional is a good option when in-person care is hard to reach.

When should I see a therapist after a heart attack or stroke? If symptoms are moderate or worse, last more than a few weeks, include flashbacks or avoidance, or affect medications and daily life. Any thoughts of self-harm need help right away: call or text 988.

Are mental health apps safe for my privacy? Not always. An analysis of top-ranked depression and smoking-cessation apps found most shared user data with third parties. Check privacy policies and prefer tools from your health system or recognised providers.
