The Medication-Nutrient Map: What Common Heart and Stroke Medicines Can Deplete

October 29, 2026•13 min read

After a heart attack, stent, bypass or stroke, most people leave with a list of medicines they'll take for years. Those medicines are the backbone of protection. Some of them also have a quieter side effect that rarely comes up in a short appointment: over time, they can lower the levels of particular nutrients.

Here is the short answer. Several common heart and stroke medicines can lower specific nutrients over time: statins lower CoQ10, loop and thiazide diuretics can lower potassium and magnesium, long-term metformin can lower vitamin B12, and long-term acid blockers can lower magnesium. Others work the opposite way: ACE inhibitors, ARBs and spironolactone can raise potassium. The safe approach is to test before supplementing, and to check every supplement against your medication list.

None of this is a reason to stop a medicine. It's a reason to know what to watch.

Statins and CoQ10

Statins block an enzyme in the liver's cholesterol pathway, the same pathway the body uses to make coenzyme Q10, a molecule involved in energy production inside cells. In a meta-analysis, statin therapy was associated with reduced circulating CoQ10 levels (Banach et al., Mayo Clinic Proceedings, 2015).

Whether that causes symptoms is less clear. Some people on statins develop muscle aches; one meta-analysis of randomised trials found CoQ10 supplementation may improve these symptoms (Qu et al., Journal of the American Heart Association, 2018), while an earlier analysis did not (Banach et al., 2015). If you have muscle symptoms, talk to your doctor before stopping your statin: a dose change, a different statin or added medicines usually solve the problem (see statins and CoQ10).

Diuretics, potassium and magnesium

Loop diuretics (such as furosemide) and thiazide diuretics (such as hydrochlorothiazide) help the kidneys remove fluid and salt, and they take potassium and magnesium with them. Low levels can cause muscle cramps, fatigue and, importantly for heart patients, rhythm problems. Most people on these medicines have regular blood tests for exactly this reason.

The opposite direction matters just as much. ACE inhibitors (such as ramipril), ARBs (such as losartan) and spironolactone or eplerenone tend to raise potassium. For people on these medicines, especially with reduced kidney function, a potassium supplement or salt substitute can push potassium dangerously high. Never start potassium on your own.

Metformin and vitamin B12; acid blockers and magnesium

Metformin, widely used for diabetes, which is common after heart attack and stroke, can reduce vitamin B12 absorption. In the Diabetes Prevention Program Outcomes Study, long-term metformin use was associated with a higher risk of B12 deficiency (Aroda et al., Journal of Clinical Endocrinology & Metabolism, 2016). Low B12 can cause tiredness, numbness or tingling, and memory problems, symptoms easily mistaken for other things after a stroke.

Proton pump inhibitors (such as pantoprazole), often prescribed alongside antiplatelet therapy to protect the stomach, can lower magnesium with long-term use; the US FDA issued a safety warning in 2011, noting the risk usually appears after more than a year. They may also reduce B12 absorption. These medicines are often important, especially with DAPT (see DAPT after a stent), so the answer is testing, not stopping.

Warfarin and vitamin K: consistency, not avoidance

Warfarin works by blocking vitamin K, so sudden changes in vitamin K intake, from leafy greens or supplements, can shift your INR. The aim isn't to avoid vitamin K but to keep intake consistent and tell your clinic about changes. Newer anticoagulants don't interact with vitamin K in this way, but they have their own interactions to check.

Some supplements also add bleeding risk with antiplatelets or anticoagulants, including high-dose fish oil, vitamin E, ginkgo, garlic and turmeric (see five drug-nutrient interactions).

How to use this map safely

  • Keep one complete list of every medicine and supplement, with doses.

  • Ask about tests that match your medicines: potassium and magnesium on diuretics, B12 on long-term metformin or acid blockers, INR on warfarin.

  • Test before you supplement, and never start potassium on your own.

  • Report symptoms such as cramps, palpitations, numbness or unusual fatigue.

  • Ask your pharmacist to check any supplement against your list.

  • Never stop a heart or stroke medicine because of a depletion concern; the protection it gives is far larger, and there's almost always a safer adjustment.

  • 服利尿剂查钾和镁,长期服二甲双胍或抑酸药查 B12,服华法林查 INR;

  • 先检测再补充,绝不自行补钾;

  • 报告痉挛、心悸、麻木或异常疲劳等症状;

  • 请药剂师核对任何补充剂;

How Healos™ supports people on long-term heart and stroke medicines

This is exactly what the Healos™ Replenish stage is built around: restoring nutrients that surgery and common heart medicines can deplete, such as CoQ10, magnesium and omega-3, in amounts matched to your medication list. The 91-Day Protocol moves through Detox, Replenish and Repair, with follow-ups at day 30 and day 60 (with a reminder to book your blood test) and a blood-test report review at day 90. Every kit is pharmacist-reviewed against your medication list before it ships, and nothing is added that conflicts with what you take, such as potassium with ACE inhibitors. It runs alongside your prescriptions, never instead of them.

The bottom line

Heart and stroke medicines protect you, and some of them can also quietly lower CoQ10, magnesium, potassium or vitamin B12, while others raise potassium. Know which ones you take, ask for the matching blood tests, test before you supplement, check everything with your pharmacist and never stop a medicine on your own.

Frequently asked questions

Do statins lower CoQ10? Yes, statins block part of the pathway the body uses to make CoQ10, and a meta-analysis found lower circulating CoQ10 on statins. Whether CoQ10 supplements help statin muscle symptoms is debated; talk to your doctor rather than stopping your statin.

Do diuretics cause low magnesium or potassium? Loop and thiazide diuretics can lower both, which is why people on them have regular blood tests. Low levels can cause cramps, fatigue and rhythm problems.

Should I take potassium with my blood pressure medicine? Not on your own. ACE inhibitors, ARBs and spironolactone can raise potassium, and adding supplements or potassium-based salt substitutes can push it dangerously high, especially with reduced kidney function.

Does metformin cause vitamin B12 deficiency? Long-term metformin use is associated with a higher risk of low B12. Ask whether periodic B12 testing is right for you, especially if you notice tiredness, numbness or memory changes.
