  • DAPT After a Stent: Why You Can't Stop Early, and When to Talk About Stepping Down

You left hospital with a new stent and two new pills that do almost the same thing: aspirin and a second antiplatelet medicine such as clopidogrel or ticagrelor. A few weeks later the bruises appear, a dentist asks whether you can stop them, or you just start to wonder whether you really need both.

Those are good questions. The answer to "can I stop early on my own?" is no, but there's a lot worth understanding about why, and about when stepping down is a conversation worth having.

Here is the short answer. After a stent, dual antiplatelet therapy, usually aspirin plus a second drug such as clopidogrel or ticagrelor, keeps platelets from clotting on the new stent while the artery heals. Stopping early without your cardiologist's advice is one of the strongest risk factors for stent thrombosis. Duration is usually six to twelve months, sometimes shorter or longer, and is decided with your cardiologist.

What does DAPT do after a stent?

A stent is a small metal scaffold pressed into the wall of a coronary artery. Until the artery's inner lining grows over it, which takes weeks to months, the metal surface can attract platelets, the blood cells that start clots. A clot inside a stent, called stent thrombosis, is uncommon but serious; it usually causes a heart attack.

Dual antiplatelet therapy (DAPT) combines two drugs that block platelets in different ways: aspirin and a P2Y12 inhibitor (clopidogrel, ticagrelor or prasugrel). Together they protect the stent far better than either alone during the healing period (see after a stent).

Why you shouldn't stop DAPT early

In a large registry of people with drug-eluting stents, premature discontinuation of antiplatelet therapy was the strongest independent predictor of stent thrombosis (Iakovou et al., JAMA, 2005). That's why cardiologists are so firm on this point, and why it's worth checking with your cardiologist, not only your dentist, family doctor or surgeon, before stopping either drug.

Common reasons people stop early include bruising, nosebleeds, cost, side effects, confusion over two similar drugs and advice from another clinician before a procedure. Each of these has a better solution than stopping: a dose or drug change, a stomach-protecting medicine, a cost-coverage review or a coordinated plan between your cardiologist and the other clinician.

How long does DAPT last?

There's no single answer; your cardiologist balances your risk of clotting against your risk of bleeding. As a general pattern, European guidance recommends about 12 months after a heart attack or other acute coronary syndrome and about 6 months after a stent for stable coronary disease (Valgimigli et al., European Heart Journal, 2018). Canadian guidance follows similar principles.

Durations can be shorter for people at high bleeding risk. In the TWILIGHT trial, stopping aspirin after three months and continuing ticagrelor alone reduced bleeding without increasing heart attacks or stroke in selected high-risk patients (Mehran et al., NEJM, 2019). Durations can be longer for some people at high clotting risk. That's the "stepping down" conversation: worth having at a planned review, never on your own.

Common side effects, and what to do instead of stopping

  • Bruising and minor bleeding (nosebleeds, bleeding gums) are common and usually not dangerous. Tell your team if they're bothersome; don't stop.

  • Stomach irritation or bleeding risk: many people are prescribed a stomach-protecting medicine (a proton pump inhibitor).

  • Shortness of breath on ticagrelor: in the PLATO trial, about 14% of people on ticagrelor reported breathlessness versus about 8% on clopidogrel (Wallentin et al., NEJM, 2009). It's usually mild and often fades, but new or worsening breathlessness should always be checked, because it can also be a heart symptom.

  • Serious bleeding such as black or bloody stools, vomiting blood, blood in urine, a severe headache or bleeding that won't stop needs urgent care. Call 911 for heavy bleeding or a sudden severe headache.

Procedures, dental work and supplements: plan ahead

Before any procedure, including dental work, colonoscopy or surgery, tell the clinician you're on DAPT and ask them to coordinate with your cardiologist. Many dental procedures can go ahead without stopping. Elective surgery is often timed after the essential DAPT period.

Supplements can add bleeding risk. High-dose omega-3 fish oil, vitamin E, ginkgo, garlic and turmeric supplements may add to the antiplatelet effect. Omega-3 is also part of the Healos™ Replenish stage, which is exactly why your medication list comes first and why you should tell your cardiologist or pharmacist about every supplement you take, ours included (see five drug-nutrient interactions).

How Healos™ supports recovery

The 91-Day Protocol moves through Clear, Replenish and Repair, providing complementary cellular nutrition and lifestyle support, with check-ins at days 30, 60 and 91 that include medication consistency and side effects, so concerns reach your cardiologist instead of leading to a quiet decision to stop. Your medication list, including antiplatelet drugs, is the first thing we ask for. Sovereign Monthly · Recovery offers the same support month to month. Neither replaces or changes your prescribed medicines.

The bottom line

Two antiplatelet pills after a stent aren't redundant; together they protect the stent while your artery heals. Don't stop either one early without your cardiologist. Bring bruising, breathlessness, cost or procedure questions to your team, tell them about every supplement, and have the stepping-down conversation at a planned review.

Frequently asked questions

How long do I need to take two antiplatelet drugs after a stent? Usually about 12 months after a heart attack or acute coronary syndrome and about 6 months after a stent for stable disease, but your cardiologist may shorten or lengthen it based on your bleeding and clotting risk.

What happens if I stop clopidogrel or ticagrelor early? Premature discontinuation of antiplatelet therapy is the strongest predictor of stent thrombosis, a clot inside the stent that usually causes a heart attack. Always check with your cardiologist first.

Why does ticagrelor make me short of breath? Breathlessness is a known side effect, reported by about 14% of people on ticagrelor in the PLATO trial. It's usually mild and often fades, but new or worsening breathlessness should be checked because it can also be a heart symptom.

Should I stop DAPT before dental work or surgery? Not without your cardiologist's input. Many dental procedures can proceed without stopping, and elective surgery is often scheduled after the essential DAPT period.

Which supplements increase bleeding risk on DAPT? High-dose fish oil, vitamin E, ginkgo, garlic and turmeric supplements may add to bleeding risk. Tell your cardiologist or pharmacist about every supplement you take.
