Sleep Problems After Heart Attack, Surgery or Stroke | Healos™
Why Sleep Falls Apart After a Heart Attack, Heart Surgery or Stroke, and How to Rebuild It
"I'm exhausted all day, and then I lie awake all night." It's one of the most common things people say in the weeks after a heart attack, heart surgery or stroke, and one of the least often addressed. Appointments focus on the heart, the brain, the medicines. Sleep gets a passing question, if that.
Here is the short answer. Broken sleep is one of the most common, and most overlooked, problems after a heart attack, heart surgery or stroke. Hospital routines, pain, medicines, sleep apnoea, anxiety and nightmares all play a part, and poor sleep in turn raises blood pressure, inflammation and low mood. The first-line treatment is not a sleeping pill but cognitive behavioural therapy for insomnia, plus checking for sleep apnoea and reviewing medicines.
Why sleep falls apart after a heart or brain event
Several things usually stack up at once:
The hospital stay. Night-time checks, alarms, light and noise disrupt the body clock, and the pattern can persist at home.
Pain and discomfort, especially after a sternotomy, when finding a comfortable position is hard for weeks.
Medicines. Some beta-blockers reduce the body's night-time melatonin; diuretics taken late cause night-time trips to the bathroom; steroids and some other medicines can be stimulating. Don't change timing yourself, but ask your pharmacist.
Sleep apnoea. It's very common: about 72% of people after stroke or TIA had sleep-disordered breathing (Johnson & Johnson, Journal of Clinical Sleep Medicine, 2010), and it's common after heart attack too (see sleep apnoea after a heart attack or stroke).
Fear, low mood and nightmares. Lying awake listening to your heartbeat, or reliving the event, keeps the stress response switched on.
Brain changes after stroke. Damage to sleep-regulating areas can directly disturb sleep; about 38% of stroke survivors have insomnia symptoms (Baylan et al., Sleep Medicine Reviews, 2020).
为什么心脏或大脑事件后睡眠会崩溃: 通常有几件事同时叠加:住院——夜间查房、警报、灯光和噪音打乱生物钟,回家后这种模式可能延续;
疼痛和不适——尤其胸骨切开后,几周里都很难找到舒服的姿势;
药物——部分 β 受体阻滞剂会减少夜间褪黑素,傍晚服用利尿剂导致夜里起夜,类固醇等药物可能有兴奋作用,不要自行调整服药时间,但可以问药剂师;
睡眠呼吸暂停——非常常见,卒中或 TIA 后约 72% 的人有睡眠呼吸障碍(2010),心梗后也很常见;
恐惧、低落和噩梦——躺着听自己的心跳或在脑中重演事件,让应激反应持续开启;
Why sleep matters for recovery
Sleep isn't a luxury in recovery; it's part of the treatment. Short sleep was associated with a 48% higher risk of developing or dying from coronary heart disease and a 15% higher risk of stroke in a large meta-analysis (Cappuccio et al., European Heart Journal, 2011). Poor sleep raises night-time blood pressure, increases inflammation, worsens blood sugar control, lowers pain tolerance and feeds depression and anxiety (see sleep debt and heart risk).
After a stroke, sleep is also when the brain consolidates what it practised in rehab, so poor sleep can slow the rewiring that recovery depends on.
What helps: CBT-I first, pills last
Cognitive behavioural therapy for insomnia (CBT-I) is the recommended first-line treatment for chronic insomnia (Qaseem et al., Annals of Internal Medicine, 2016). It usually takes four to eight sessions, in person, online or through a guided programme, and works by resetting the habits and thoughts that keep insomnia going. Its core steps include:
A fixed wake-up time, every day, even after a bad night.
Bed for sleep only; if you're awake for more than about 20 minutes, get up and do something calm until sleepy.
Matching time in bed to actual sleep at first, then gradually extending it.
Working with worried thoughts about sleep and about your heart.
Alongside CBT-I: get tested for sleep apnoea if you snore, stop breathing in your sleep or wake unrefreshed; ask for a medication review of timing and doses; treat pain properly; and address fear or nightmares with your care team.
About sleeping pills: some are useful short-term, but many sedatives slow breathing, increase falls and confusion, and are dangerous combined with opioid painkillers or alcohol (see the danger of mixing opioids and sedatives). Use them only as prescribed, for the shortest time, and never with alcohol.
每天固定起床时间,即使前一晚没睡好;
床只用来睡觉,醒着超过约 20 分钟就起来做点平静的事,困了再回床;
起初让卧床时间与实际睡眠时间匹配,再逐步延长;
A simple plan for tonight
Same wake-up time tomorrow, whatever happens tonight.
Morning daylight for 20 to 30 minutes.
Caffeine only before noon; little or no alcohol.
A wind-down hour: dim lights, screens away, something calm.
A cool, dark, quiet bedroom.
If you're awake for more than 20 minutes, get up until sleepy.
Write worries down before bed, and bring them to your next appointment.
无论今晚睡得如何,明天同一时间起床;
早晨晒 20–30 分钟太阳;
咖啡因只在中午前,少喝或不喝酒;
留一小时放松:调暗灯光、放下屏幕、做点平静的事;
卧室凉爽、黑暗、安静;
醒着超过 20 分钟就起床,困了再回去;
How Healos™ supports recovery
In the Healos™ framework, sleep sits inside the mind-body-spirit multiplier: when sleep improves, every other part of recovery works better. The 91-Day Protocol moves through Detox, Replenish and Repair, providing complementary cellular nutrition matched to your medication list, with follow-ups at day 30 and day 60 (with a reminder to book your blood test) and a blood-test report review at day 90 that track sleep alongside mood and your numbers. Sovereign Monthly · Recovery continues month to month. Neither replaces CBT-I, sleep apnoea treatment or your care team.
The bottom line
Sleep often falls apart after a heart attack, heart surgery or stroke, for good reasons: the hospital, pain, medicines, sleep apnoea, fear and brain changes. It matters, because poor sleep raises blood pressure, inflammation and low mood. Start with CBT-I and a steady wake-up time, get checked for sleep apnoea, review your medicines and treat sleeping pills as a short-term last resort.
