  • Home After a Stroke: The First Four Weeks Nobody Explains

The day of discharge is often a blur: a stack of prescriptions, a list of appointments, a nurse explaining things quickly, and a family member trying to remember it all. Then the door closes and you're home, with no call button and no one checking in at 2 a.m.

Those first weeks matter more than most families realise.

Here is the short answer. The first four weeks home after a stroke are when readmissions, falls and missed medications are most likely. About one in seven people is readmitted within 30 days, falls are very common in the first months, and about a quarter stop at least one prevention medication within three months. A week-by-week plan covering medications, safety, follow-up, fatigue and mood makes the transition safer.

Why the first month home is a high-risk window

  • Readmissions: about 14% of Medicare patients were readmitted within 30 days after an ischaemic stroke (Lichtman et al., Stroke, 2010), often for infections, falls, heart problems or another stroke.

  • Falls: weakness, balance problems, vision changes, fatigue and new medications combine at home. In one study, 73% of stroke survivors fell within six months of going home (Forster & Young, BMJ, 1995).

  • Medications: about one in four patients stopped one or more prevention medications within three months (Bushnell et al., Archives of Neurology, 2010), usually because of confusion, side effects or cost.

  • Another stroke: the risk of a repeat stroke is highest in the early weeks and months, which is why blood pressure, antiplatelet or anticoagulant therapy and statins matter so much right now (see blood pressure after a stroke).

  • 再入院:约 14% 的联邦医保患者在缺血性卒中后 30 天内再入院(2010),常因感染、跌倒、心脏问题或再次卒中;

  • 跌倒:无力、平衡问题、视力变化、疲劳和新药物在家中叠加,一项研究中 73% 的卒中幸存者在回家后六个月内跌倒过(BMJ 1995);

  • 药物:约四分之一的患者三个月内停用了一种或多种预防药物(Arch Neurol 2010),通常因为困惑、副作用或费用;

A week-by-week guide to the first month

Week 1: medications and safety. Sort every medication into a weekly pill organiser or pharmacy blister pack, and make one clear list with doses and reasons. Ask the pharmacist to review it. Do a quick home safety sweep: clear walkways, remove loose rugs, add night lights, put essentials within reach and consider a shower chair and grab bars. Confirm every follow-up appointment is booked.

Week 2: rhythm and rehab. Settle into a daily routine: medications at the same times, short rehab exercises spread through the day, meals, rest. Expect fatigue; plan rest before exhaustion (see post-stroke fatigue). Start home blood pressure readings if your team has asked.

Week 3: mood and sleep. Watch for low mood, tearfulness, irritability or loss of interest: about 31% of survivors experience depression. Notice snoring or breathing pauses at night, since sleep apnoea is very common after stroke. Both are treatable; mention them at the next appointment.

Week 4: review and look ahead. Bring a list to your follow-up: blood pressure readings, side effects, falls or near-falls, mood, questions about returning to work or driving. Driving rules vary by province, and many require a medical assessment before you drive again. Ask about outpatient or community rehab if it hasn't started.

For families and caregivers

Caregivers often carry the discharge instructions, the medication schedule and the worry. A few things help:

  • Write it down. One shared notebook or note on your phone for medications, appointments, readings and questions.

  • Encourage, don't take over. Doing things for someone is faster, but practising everyday tasks is how the brain rewires (see what happens to your brain after a stroke).

  • Know the warning signs. New weakness, speech or vision problems, a severe headache, confusion or a fall with a head injury need urgent help.

  • Look after yourself. Caregiver exhaustion is common. Accept help, take breaks and ask about local caregiver support.

Frequently asked questions

What should I expect in the first week home after a stroke? Expect fatigue, a lot of new medications and several follow-up appointments. Focus on organising medications, making the home safer and confirming every appointment is booked.

How can I prevent falls after a stroke? Clear walkways, remove loose rugs, add night lights and grab bars, use a shower chair, keep essentials within reach, stand up slowly and ask your rehab team about balance exercises. Falls are very common in the first months home.

Why do people stop stroke medications after discharge? About one in four stop at least one prevention medication within three months, often because of confusion, side effects or cost. A single clear medication list, a pill organiser and a pharmacist review help.
