  • Senior Cardiac Nutrition Depletion: The Silent Burden

#LTC, #Senior Nutrition, #Malnutrition, #Vitamin B12, #Vitamin D, #Magnesium, #Protein, #Heart Failure

Malnutrition in older cardiac patients rarely looks dramatic. It looks like half a lunch left on the plate, a resident who is "just a bit more tired lately", a slow slide in weight that nobody quite notices because clothes are loose anyway. It is quiet, cumulative and very easy to attribute to age.

The short answer: older adults with heart disease face a double squeeze on nutrition. Ageing reduces appetite, thirst and absorption of nutrients such as vitamin B12, while cardiac medications add predictable losses of magnesium, potassium, B12 and coenzyme Q10. About 45% of adults admitted to Canadian hospitals are malnourished, and long-term care residents are at similar risk, so routine screening matters.

The first squeeze: what ageing does to nutrition

Appetite and taste. Appetite declines with age, a pattern sometimes called the anorexia of ageing. Taste and smell dull, dentures and dry mouth make eating harder, and medications can alter taste further.

Absorption. Between 10 and 30% of older adults have atrophic gastritis or reduced stomach acid, which impairs absorption of vitamin B12 from food. For that reason, adults over 50 are advised to get most of their B12 from fortified foods or supplements rather than relying on diet alone (Institute of Medicine, 1998).

Thirst. The sense of thirst weakens with age, so older adults often drink less than they need without feeling thirsty, which matters a great deal for anyone on a diuretic.

Muscle. Older adults need more protein, not less, to maintain muscle. The PROT-AGE group recommends 1.0 to 1.2 g per kilogram of body weight daily for healthy older adults, and 1.2 to 1.5 g/kg for those with acute or chronic illness (Bauer et al., JAMDA, 2013), unless kidney disease requires otherwise. Many residents fall well short.

Swallowing. Dysphagia is common after stroke and in advanced frailty, and texture-modified diets can reduce both intake and nutrient density if not carefully planned.

The second squeeze: what cardiac medications add

Layered on top of ageing is the medication list. The average cardiac resident takes several drugs with known nutritional effects (see the drug-nutrient debt for the clinician's version).

  • Loop and thiazide diuretics increase urinary losses of magnesium and potassium, and in combination with reduced thirst they raise the risk of dehydration.

  • Proton pump inhibitors, common in residents on antiplatelet therapy, can lower magnesium with long-term use, and two or more years of use was associated with 65% higher odds of B12 deficiency (Lam et al., JAMA, 2013), compounding the absorption problem of ageing.

  • Metformin, for residents with diabetes, further lowers B12.

  • Statins lower coenzyme Q10, which may contribute to muscle symptoms and fatigue in some residents.

  • Low-dose aspirin and anticoagulants can contribute to iron deficiency through small, unnoticed blood losses.

Each effect is modest on its own. Combined with poor intake and reduced absorption, they add up to a burden that shows up as fatigue, weakness, falls, confusion or arrhythmias.

The salt paradox in frail cardiac residents

Low-sodium diets are standard advice for heart failure and high blood pressure. In frail older adults with poor appetite, though, a strict low-salt menu can make food so unappealing that total intake falls, and inadequate protein and calories may do more harm than a moderate amount of salt.

This isn't an argument against sodium restriction. It is an argument for individualising it with the physician and dietitian, especially for residents who are losing weight. The goal is a diet the resident will actually eat. Herbs, spices, acidity and texture can do a lot of the work that salt used to do.

Salt substitutes deserve a specific warning. Most are based on potassium chloride, and residents taking ACE inhibitors, ARBs or spironolactone, particularly with reduced kidney function, can develop high potassium if they use them freely.

A 10-minute monthly nutrition check for cardiac residents

Screening doesn't need to be elaborate to be useful. A short monthly routine, shared between nursing, dietary and pharmacy staff, catches most problems early.

  1. Weight trend. Compare this month with last month and with three months ago. Unplanned loss of 5% or more in a month, or 10% over six months, is a common trigger for dietitian review. In heart failure residents, remember that fluid can mask muscle loss.

  2. Meal and fluid intake. Note residents regularly leaving more than a quarter of their meals, and anyone whose fluid intake is falling while on a diuretic.

  3. Medication changes. Any new diuretic, PPI, metformin or dose change is a prompt to check electrolytes and review nutrition.

  4. Energy and function. New fatigue, weakness, falls or confusion deserve a nutrition question as well as a medical one.

  5. Supplements and salt substitutes. Ask families what they bring in. Well-meaning gifts of supplements or potassium-based salt substitutes are a common blind spot.

Most homes already collect much of this information. The gain comes from reviewing it together, on a schedule, with the medication list in view.

How Healos™ supports nutrition in LTC

Healos™ supports long-term care teams as complementary help alongside dietitians, nurses, physicians and consultant pharmacists. For LTC partners, every resident plan starts with the medication list, so predictable drug-nutrient effects and supplement interactions are flagged for the pharmacist before anything is added. Structured resident summaries built on the H-Index help teams track intake, energy, mood and function over time, and materials are available in English and Chinese for residents and families.

Healos™ nutritional products are Health Canada NPN-licensed and positioned as complementary support, never as treatment for malnutrition or any medical condition. Clinical decisions, including any supplementation, remain with the resident's care team.

Healos™

The bottom line

Nutrient depletion in older cardiac patients is silent because every piece of it looks like ageing. Put together, reduced appetite and absorption plus predictable medication effects create a burden that affects strength, cognition, heart rhythm and readmission risk. A short, regular screening routine makes the invisible visible, while there is still time to act.

Frequently asked questions

Why are older heart patients at risk of malnutrition? Ageing reduces appetite, thirst and nutrient absorption, and cardiac medications add losses of magnesium, potassium, vitamin B12 and coenzyme Q10. Low-salt diets, dental problems and swallowing difficulties can further reduce intake.

How much protein do older adults need? The PROT-AGE group recommends 1.0 to 1.2 g of protein per kg of body weight daily for healthy older adults and 1.2 to 1.5 g/kg for those with acute or chronic illness, unless kidney disease requires a different target set by their care team.

Why do older adults need vitamin B12 from fortified foods or supplements? Between 10 and 30% of older adults absorb food-bound vitamin B12 poorly because of reduced stomach acid. Long-term proton pump inhibitors and metformin can lower B12 further, so checking levels in older cardiac patients is worthwhile.

Should seniors in long-term care take vitamin D? Osteoporosis Canada recommends 800 to 2,000 IU of vitamin D daily for adults over 50, and supplementation is common in long-term care because residents get little sun exposure. The resident's care team should confirm the appropriate amount.

Do diuretics cause nutrient deficiencies in older adults? Loop and thiazide diuretics increase magnesium and potassium losses, and combined with a weaker sense of thirst they raise the risk of dehydration. Regular monitoring of electrolytes, weight and fluid intake is important.
