  • Statins, Diuretics, and DAPT: The Drug-Nutrient Debt (For Clinicians)

Every drug on a standard post-cardiac discharge list earns its place. Statins, antiplatelet therapy, beta-blockers, ACE inhibitors and diuretics each have a strong evidence base for secondary prevention, and nothing in this article argues for changing them.

What they also have is a nutritional side ledger. Month after month, predictable effects on coenzyme Q10, magnesium, potassium, vitamin B12 and iron accumulate quietly, and they surface at 6, 12 or 18 months as fatigue, cramps, palpitations, low mood or declining exercise tolerance that doesn't quite fit the clinical picture.

The short answer for busy clinicians: standard post-cardiac medications have predictable nutritional side effects. Statins lower coenzyme Q10, loop and thiazide diuretics increase magnesium and potassium loss, long-term proton pump inhibitors used with DAPT can lower magnesium and vitamin B12, metformin lowers B12, and low-dose aspirin can contribute to iron deficiency. ACE inhibitors and aldosterone antagonists do the opposite with potassium.

Statins and coenzyme Q10: real depletion, contested remedy

Statins inhibit HMG-CoA reductase, the same mevalonate pathway step that leads to coenzyme Q10 synthesis. The effect on circulating levels is well documented: high-dose atorvastatin lowered blood CoQ10 by about half within 30 days (Rundek et al., Archives of Neurology, 2004). Whether muscle CoQ10 falls to a clinically meaningful degree is less clear.

The treatment question is genuinely contested. A 2018 meta-analysis found that CoQ10 supplementation reduced statin-associated muscle symptoms (Qu et al., Journal of the American Heart Association, 2018), while an earlier one did not (Banach et al., Mayo Clinic Proceedings, 2015). The pragmatic position: in a patient with muscle symptoms who is at risk of discontinuing a statin, CoQ10 is a low-risk option worth discussing, provided it keeps the patient on the statin rather than replacing it. The patient-facing version of this discussion is in statins deplete CoQ10.

Diuretics: magnesium, potassium and a calcium twist

Loop and thiazide diuretics increase urinary losses of magnesium and potassium. Low magnesium is associated with arrhythmias and makes hypokalaemia harder to correct, because potassium repletion is less effective while magnesium remains low. In patients on long-term diuretics, particularly with heart failure or atrial fibrillation, periodic magnesium measurement alongside potassium is worthwhile, recognising that serum magnesium underestimates total body stores.

Thiazides add a twist: they reduce urinary calcium excretion. In patients also taking calcium and vitamin D supplements, which is common in older women, that combination occasionally produces hypercalcaemia. It is worth knowing what the patient is taking over the counter.

DAPT's hidden companions: PPIs, iron and bleeding

Antiplatelet therapy itself doesn't deplete many nutrients, but it brings two companions.

Proton pump inhibitors. PPIs are often co-prescribed for gastrointestinal protection during DAPT. In 2011, the US FDA warned that long-term PPI use, generally over a year, can cause hypomagnesaemia. Two or more years of PPI use has also been associated with a 65% higher odds of vitamin B12 deficiency (Lam et al., JAMA, 2013). The PPI is often continued long after DAPT has been stepped down; each review is an opportunity to ask whether it's still needed. (Where clopidogrel is used, pantoprazole avoids the CYP2C19 interaction associated with omeprazole and esomeprazole.)

Iron. Low-dose aspirin causes small amounts of occult gastrointestinal blood loss. In the ASPREE trial of older adults, aspirin increased incident anaemia by about 20% and was associated with lower ferritin (McQuilten et al., Annals of Internal Medicine, 2023). In patients on DAPT or aspirin plus an anticoagulant, unexplained fatigue warrants a ferritin as well as a haemoglobin.

Bleeding-additive supplements. Patients on antiplatelets often add high-dose fish oil, vitamin E, ginkgo, turmeric or garlic extracts without mentioning it. Individually the effects are usually modest, but they add to bleeding risk and are rarely recorded.

Metformin, beta-blockers and the potassium surplus

Metformin. Many post-cardiac patients have type 2 diabetes. Long-term metformin use was associated with vitamin B12 deficiency in the Diabetes Prevention Program Outcomes Study (Aroda et al., Journal of Clinical Endocrinology & Metabolism, 2016). Combined with a PPI, the risk compounds, and B12 deficiency can present as neuropathy, cognitive complaints or fatigue that is easily attributed to something else.

Beta-blockers and sleep. Some beta-blockers reduce nocturnal melatonin secretion (Stoschitzky et al., European Journal of Clinical Pharmacology, 1999). It is one plausible contributor when patients report new insomnia after starting therapy, and worth considering before adding a sedative.

The opposite problem: potassium surplus. ACE inhibitors, ARBs and mineralocorticoid receptor antagonists raise potassium. Patients who add potassium supplements or switch to potassium-based salt substitutes, often on well-meaning advice to reduce sodium, can develop hyperkalaemia, especially with reduced kidney function. The medication list and the grocery list need to be read together.

A practical screening rhythm

None of this requires a new clinic. A simple rhythm covers most of it:

  • At each medication review, record over-the-counter supplements and salt substitutes as carefully as prescriptions.

  • Check magnesium with potassium in patients on long-term diuretics or PPIs.

  • Check B12 in patients on metformin, long-term PPIs, or both, and in anyone with new neuropathy or cognitive complaints.

  • Add ferritin when patients on antiplatelet therapy report unexplained fatigue.

  • Ask about muscle symptoms before they become a reason to stop a statin.

  • Reassess whether a PPI is still indicated once DAPT is stepped down.

Healos supports this rhythm from the patient side. Every programme starts with the medication list, flags predictable drug-nutrient effects and possible supplement interactions in the H-Index pre-visit summary, and routes anything that needs clinical judgement back to the prescriber. Healos™ products are Health Canada NPN-licensed and positioned as complementary support; prescribing decisions stay with you.

  • 每次药物复查时,像记录处方一样认真记录非处方补充剂和代盐;

  • 长期使用利尿剂或 PPI 者,测钾时一并测镁;

  • 服用二甲双胍、长期 PPI 或两者兼有者,以及新发神经病变或认知问题者,检查 B12;

  • 服用抗血小板药的患者出现不明原因疲劳时加查铁蛋白;

  • 在肌肉症状成为停用他汀的理由之前主动询问;

The bottom line

The post-cardiac prescription list is correct. It also runs up a predictable nutritional debt that rarely appears on anyone's checklist until it shows up as a symptom. Naming that debt early, and screening for it on a simple rhythm, keeps patients on the medications that protect them.

Frequently asked questions

Which cardiac medications deplete nutrients? Statins lower coenzyme Q10; loop and thiazide diuretics increase magnesium and potassium loss; long-term proton pump inhibitors can lower magnesium and vitamin B12; metformin lowers B12; and low-dose aspirin can contribute to iron deficiency through occult blood loss.

Should patients on statins take CoQ10? Evidence is mixed. Statins lower circulating CoQ10, and one meta-analysis found supplementation reduced statin-associated muscle symptoms while another did not. For patients at risk of stopping a statin because of muscle symptoms, CoQ10 is a low-risk option to discuss, as long as the statin continues.

Do diuretics cause low magnesium? Yes. Loop and thiazide diuretics increase urinary magnesium and potassium losses. Low magnesium is associated with arrhythmias and makes low potassium harder to correct, so periodic monitoring is reasonable in long-term users.

Does long-term PPI use cause vitamin B12 deficiency? It can. Two or more years of proton pump inhibitor use was associated with 65% higher odds of B12 deficiency in a large JAMA study, and long-term use can also cause low magnesium. Reassessing the need for a PPI after DAPT is stepped down is worthwhile.

Can low-dose aspirin cause iron deficiency? It can contribute. In the ASPREE trial, low-dose aspirin increased incident anaemia by about 20% in older adults and was associated with lower ferritin, likely through small amounts of gastrointestinal blood loss.
