No Symptoms After Heart Surgery Isn't No Risk | Healos™
No Symptoms Does Not Mean No Risk: Why Stability Is the Most Critical Window
There's a particular kind of relief that arrives about six months after a heart attack or stent. The fatigue has lifted. The follow-up appointments are further apart. You've stopped listening to your own heartbeat at night. Friends say you look like yourself again, and you agree.
Enjoy that. It's earned. But it is also the moment when the most important part of long-term recovery quietly begins, because the absence of symptoms and the absence of risk are two very different things.
Here is the short answer. Feeling well 6 to 18 months after a heart attack or stent is good news, but it doesn't mean risk is gone. About 1 in 5 survivors has another major cardiovascular event in the first year, and risk stays elevated for years. Many heart attacks cause no clear symptoms, and adherence to protective medications often slips once people feel fine.
Can you have another heart attack without symptoms?
Yes, and more often than most people realise. In the long-running ARIC study, about 45% of heart attacks were silent: they caused no symptoms clear enough to send the person to hospital and were discovered only on a later ECG (Zhang et al., Circulation, 2016). Silent heart attacks still damage heart muscle and still raise the risk of future events.
The same logic applies to the processes that lead to a heart attack. Plaque growth, rising blood pressure, creeping LDL cholesterol and chronic inflammation make no noise. As the 10-year cellular timeline shows, atherosclerosis spends most of its life completely silent. Feeling good tells you that your heart is coping well today. It can't tell you what's happening in your arteries.
What is the risk of another heart attack after the first year?
The best real-world picture comes from a Swedish national registry of more than 97,000 heart attack survivors. In the first year, about 18% had another heart attack, a stroke or cardiovascular death. The finding that surprises people is what came next: among those who made it through the first year without any event, about 20% had one over the following three years (Jernberg et al., European Heart Journal, 2015).
In other words, getting through year one safely is a real achievement, but it doesn't reset your risk to that of someone who never had heart disease. The risk curve flattens; it doesn't disappear. That is why cardiologists talk about lifelong secondary care, and why the stable phase deserves as much structure as the months right after the procedure.
Why the stable phase is when people drift
Here's the uncomfortable part. The months when people feel best are often the months when protective habits quietly erode.
Medication adherence is the clearest example. When researchers followed older adults in Ontario after an acute coronary syndrome, only about 40% were still taking their statin two years later (Jackevicius et al., JAMA, 2002). Few of those people made a deliberate decision to stop. Prescriptions ran out and weren't renewed. A side effect was blamed on the pill. The pill stopped feeling necessary because nothing hurt.
Other habits follow the same curve. The daily walk becomes three times a week, then weekends. Salt creeps back. Appointments are spaced further apart, so there are fewer moments when anyone checks. Psychologists sometimes call this a false sense of safety: the brain reads "no symptoms" as "problem solved".
None of this is a character flaw. It is exactly what you'd expect when the reward for effort is invisible. Something that doesn't hurt is hard to keep treating unless you have a way to see that it still matters.
The risk markers you can't feel, and how to track them
If symptoms aren't a reliable guide, what is? A handful of measurable markers give a far better picture of how your arteries are doing. Most are routine; a few are worth asking about.
LDL cholesterol. The single most important number for most people after a heart attack. Canadian guidelines use 1.8 mmol/L as the threshold for intensifying treatment after a heart attack (Pearson et al., Canadian Journal of Cardiology, 2021).
Blood pressure. Home readings, taken correctly and regularly, are more informative than an occasional clinic measurement.
hs-CRP. High-sensitivity C-reactive protein reflects low-grade inflammation. Inflammation carries its own risk: in the CANTOS trial, lowering it without changing cholesterol reduced repeat events by about 15% (Ridker et al., NEJM, 2017).
Lipoprotein(a). A largely inherited cholesterol particle that raises risk even when LDL is controlled. Canadian guidelines recommend measuring it once in adulthood. Many heart attack survivors have never had it checked.
Blood sugar, kidney function and waist size. Each shifts slowly and each changes your risk.
Mood and sleep. Depression and poor sleep are linked to worse outcomes and to slipping adherence. They are risk markers too, even though no blood test shows them.
Tracking these over time turns an invisible process into something you and your cardiologist can actually see and act on.
Repairing versus protecting: why this phase needs a different approach
The first months after a cardiac event are about repair: settling inflammation, rebuilding strength, adjusting medications, restoring nutrients that surgery and new prescriptions disturbed. That work has a beginning and an end.
The stable phase is different. The goal isn't to fix something that just happened; it's to keep the same conditions from slowly rebuilding. That calls for lower-intensity, longer-term support: steady maintenance nutrition matched to your medications, regular check-ins that make hidden markers visible, and a structure that keeps habits from drifting when nothing hurts.
That is what Sovereign Monthly · Protect is designed for. It provides monthly maintenance nutrition built around four cellular nutrients, adjusted to your diagnosis and medication list and pharmacist-reviewed before every kit ships, with regular check-ins that keep your numbers, sleep and habits in view and produce a summary for your cardiologist. It is complementary support alongside your medications and specialist follow-up, never a replacement for them, and it can be cancelled at any time.
Between fear and complacency: finding the right level of attention
There are two ways to get the stable phase wrong, and they pull in opposite directions.
The first is complacency: treating the heart attack as a closed chapter, letting medications lapse and assuming that feeling fine means being fine. The second is fear: checking your pulse constantly, avoiding exercise, reading every twinge as the start of another event. Fear is exhausting, and it backfires. People who are frightened often move less, sleep worse and find it harder to stick with routines, which quietly raises the very risk they're worried about.
The healthy middle is structured attention. You don't need to think about your heart every hour. You need a small number of regular checkpoints that tell you, reliably, whether things are on track: a monthly review of mood, sleep and activity, lab results at the intervals your team recommends, and a clear plan for which symptoms mean "call today" and which mean "mention it at the next visit".
That structure does something important psychologically. It moves the job of worrying from you to the system. When the checkpoints look good, you can genuinely relax, because you have evidence rather than hope. When something shifts, it gets caught early, while it's still easy to address.
Families play a role here too. Partners often swing between the same two extremes, either hovering or assuming the danger has passed. Sharing the checkpoints with them gives everyone the same, calmer picture of how things are going.
Your stability checklist: six things to keep doing when you feel fine
Keep taking every prescribed medication, and renew prescriptions before they run out. If a side effect bothers you, talk to your doctor or pharmacist about alternatives rather than stopping.
Know your numbers: LDL, blood pressure, blood sugar and kidney function, and ask about hs-CRP and a one-time lipoprotein(a) test.
Protect your daily movement with a fixed time and a minimum you never skip, even on bad days.
Keep appointments even when you feel well. Those are the visits where silent changes get caught.
Check in on mood and sleep at least monthly, and tell your team if either slips.
Know the warning signs and call 911 for chest pain or pressure, new breathlessness, fainting or any sign of stroke, even if you've felt fine for months.
The bottom line
Feeling fine after a heart attack or stent is the reward for everything you did in the first months. Protecting that feeling is the work of the next years. Risk after a cardiac event flattens but doesn't vanish, and it rarely announces itself. The people who do best are the ones who keep a quiet structure in place long after the symptoms are gone.
Frequently asked questions
I feel fine after my heart attack. Do I still need to worry about my risk? Feeling well is a good sign, but risk remains elevated for years. In a Swedish registry, about 18% of survivors had another major cardiovascular event in the first year, and about 20% of those who were event-free after year one had an event in the following three years. Continuing medications and follow-up matters even when you feel fine.
Can you have a heart attack without symptoms? Yes. In the ARIC study, about 45% of heart attacks were silent and were discovered only on a later ECG. Silent heart attacks still damage heart muscle and raise future risk.
Why do people stop taking statins after a heart attack? Often without deciding to: prescriptions lapse, side effects are blamed on the medication, or it stops feeling necessary once there are no symptoms. In Ontario, only about 40% of older adults were still taking a statin two years after an acute coronary syndrome. Talk to your doctor before stopping any medication.
What tests should I have a year after a stent or heart attack? Common checks include LDL cholesterol, blood pressure, blood sugar and kidney function. It is also reasonable to ask about hs-CRP, a marker of inflammation, and a one-time lipoprotein(a) test, which Canadian guidelines recommend once in adulthood.
What is the difference between cardiac repair and long-term protection? Repair focuses on the months right after a cardiac event: settling inflammation, rebuilding strength and adjusting medications. Long-term protection focuses on keeping risk factors from slowly rebuilding through steady habits, maintenance nutrition matched to medications, and regular tracking of risk markers.
