Heart Attack Causes: The 10-Year Cellular Timeline
It felt like it came out of nowhere. A tightness in the chest on an ordinary morning, an ambulance, a catheter threaded through your wrist, and within about an hour the blocked artery was open again. The whole emergency, from first symptom to stent, may have taken less time than a long lunch.
That speed is one of modern medicine's great achievements. It is also the source of a common and costly misunderstanding: that the heart attack was an event of that morning, and that the stent therefore fixed it.
Here is what actually happened. A heart attack usually happens in minutes, but the disease behind it, atherosclerosis, typically builds silently over a decade or more. It begins with damage to the artery lining, followed by cholesterol buildup, inflammation and plaque formation. A stent opens one blocked artery; it does not remove the process affecting the rest of the arteries.
Understanding that timeline is not meant to frighten you. It is the most useful reframe you can have in recovery, because the same slow biology that built the problem can be pushed in the other direction.
How long does atherosclerosis take to develop?
Much longer than most people realise. In 1953, pathologists examining young American soldiers killed in the Korean War, whose average age was 22, found visible coronary atherosclerosis in 77% of them (Enos et al., JAMA, 1953). Later research, including the large PDAY study of adolescents and young adults, confirmed that fatty streaks in the arteries are common by the teens and twenties.
Fast forward to middle age. The PESA study used ultrasound and CT imaging on 4,184 office workers aged 40 to 54 who had no heart symptoms at all. 63% already had plaque somewhere in their arteries (Fernández-Friera et al., Circulation, 2015).
In other words, for most people who have a heart attack in their 50s or 60s, the disease had been present, quietly and without symptoms, for many years. The "10-year timeline" in this article is a simplified way to describe that process. For some people it is shorter, for many it is longer, and genetics, diabetes, smoking and blood pressure all change the pace.
The 10-year cellular timeline, stage by stage
Stage 1: The artery lining is injured (the early years)
Your arteries are lined with a single layer of cells called the endothelium. Healthy endothelium keeps blood vessels relaxed, smooth and resistant to clotting and inflammation. High blood pressure, smoking, high blood sugar, high LDL cholesterol, chronic stress and inflammation all wear it down. This state, called endothelial dysfunction, can be measured years before any plaque causes symptoms, and it predicts future cardiovascular events.
Stage 2: Cholesterol gets trapped and oxidised
When the lining becomes leaky and sticky, LDL particles slip into the artery wall and become trapped. There, they are chemically altered by oxidation. The immune system treats oxidised LDL as a threat and sends white blood cells called monocytes into the wall.
Stage 3: Foam cells and the first fatty streak
Monocytes turn into macrophages and swallow oxidised LDL until they are swollen with fat. Under a microscope they look foamy, hence the name foam cells. Clusters of foam cells form a fatty streak, the earliest visible lesion. At this stage nothing is blocked and nothing hurts.
Stage 4: A plaque grows, and the artery hides it
Over years, smooth muscle cells migrate in and build a fibrous cap over a core of fat and dead cells. Calcium may deposit. Remarkably, the artery often expands outward to make room, a process known as Glagov remodeling, so the channel for blood stays open. This is why a significant plaque can exist without chest pain and why a stress test can be normal.
Stage 5: The plaque becomes unstable
Inflammation inside the plaque releases enzymes that thin the fibrous cap. A plaque with a large soft core and a thin cap is described as vulnerable. It may not narrow the artery much at all.
Stage 6: Minutes, not years
A thin cap cracks or the surface erodes. Blood meets the plaque's contents, and a clot forms within minutes. If the clot blocks the artery, heart muscle downstream begins to die. This is the heart attack: the final minutes of a story that took years to write.
Why many heart attacks start in plaques that "weren't that bad"
Classic studies of heart attacks found that the plaque responsible often had not been severely narrowing the artery beforehand (Falk, Shah & Fuster, Circulation, 1995). Severe narrowings tend to cause predictable chest pain with exertion. Moderate but inflamed plaques can be silent right up until they rupture. This is why "no symptoms" is not the same as "no disease," and why doctors focus on stabilising all plaques with medication rather than only stenting the tightest one.
Does a stent fix heart disease?
A stent does something vital. During a heart attack, opening the blocked artery quickly saves heart muscle and saves lives. That benefit is real and well established.
What a stent does not do is treat the other plaques in your arteries or reverse the conditions that created them. The clearest evidence comes from people with stable coronary disease. In the COURAGE trial (Boden et al., NEJM, 2007) and later the ISCHEMIA trial of 5,179 patients (Maron et al., NEJM, 2020), adding stents or bypass to optimal medical therapy did not reduce heart attacks or death overall compared with medication and lifestyle treatment alone, although procedures did help relieve angina. The disease is systemic, so the treatment has to be systemic too.
That is why, after a successful stent, your medications matter so much. About 18% of heart attack survivors have another major cardiovascular event within the first year (Jernberg et al., European Heart Journal, 2015). Statins, antiplatelet drugs, blood pressure control and cardiac rehab are the backbone of preventing that. Never stop them without your cardiologist's advice. If you are on a statin and struggling with fatigue or muscle aches, read about why statins deplete CoQ10 and bring it up with your prescriber.
Can atherosclerosis be reversed after a stent?
Partly, and more importantly, it can be stabilised. The honest summary of the research is this:
Plaque can shrink, but modestly. In the GLAGOV trial, adding the cholesterol-lowering drug evolocumab to a statin reduced plaque volume by about 0.95 percentage points over 78 weeks, compared with almost no change on statin alone (Nicholls et al., JAMA, 2016). Small numbers, but a measurable shift in direction.
Plaque can become safer. Intensive treatment tends to thicken the fibrous cap and shrink the soft, inflamed core, making rupture less likely even when overall size changes little.
The artery lining responds faster. Endothelial function can improve within weeks to months of stopping smoking, regular aerobic exercise, blood pressure control and lipid lowering. This is where daily life has the most leverage.
Inflammation is a target of its own. High-sensitivity C-reactive protein (hs-CRP) is a blood marker of inflammation. Persistently elevated levels signal risk that remains even when LDL is controlled, which is why many cardiologists now track it.
What the 10-year timeline means for your recovery
If a heart attack is the last few minutes of a decade-long process, then recovery cannot be measured only in the days after the stent. It has to work on the same levels where the disease was built: the artery lining, lipids, inflammation, oxidative stress, blood sugar, sleep, stress and daily habits.
This is the thinking behind the Healos™ 91-Day Protocol. Ninety-one days is not a cure and is not meant to undo ten years. It is the first structured cycle of work on that environment, designed as complementary nutritional and lifestyle support alongside your cardiologist's plan. It moves through three stages, Detox, Replenish and Repair: first reducing oxidative load, then replacing nutrients commonly affected by surgery and cardiac medications, then supporting the body’s own repair and immune balance. Every kit is pharmacist-reviewed against your medication list, with follow-ups at day 30 and day 60 and a blood-test report review at day 90. The biology behind that length is covered in the 91-day cellular renewal window.
One more factor shapes all of this: how you feel. Fear, low mood and chronic stress raise stress hormones, disturb sleep and erode the daily habits that protect your arteries. In the Healos™ model, the mind-body-spirit state acts as a multiplier on every other effort. If you are struggling emotionally, that is part of your cardiac recovery, not separate from it (see why you may still feel unwell after a successful procedure).
Five questions to ask your cardiologist at your next visit
What is my LDL cholesterol target, and am I there? (Canadian guidelines use 1.8 mmol/L as the level at which to intensify therapy after a heart attack.)
Should my lipoprotein(a) be measured? Canadian guidelines recommend testing it once in adulthood.
Would an hs-CRP test add useful information in my case?
Am I referred to cardiac rehab, and what happens after it ends?
What blood pressure and blood sugar targets should I aim for?
The free Healos™ H-Index™ can help you prepare. It is a pre-visit communication tool, not a diagnostic test, and it turns your recovery across 11 areas into a structured summary you can bring to that appointment.

The bottom line
Your heart attack was the final scene, not the whole story. The stent solved the emergency. The years that led to it are still written into your arteries, and the next years are still being written. That is the hopeful part: the same slow, cellular processes that built the problem respond to steady, sustained work in the opposite direction, alongside the medical care that keeps you safe.
Frequently asked questions
How long does atherosclerosis take to develop? Usually many years. Fatty streaks can appear in the teens and twenties, and imaging studies show that most middle-aged adults without symptoms already have some plaque. A heart attack in midlife typically reflects a process that has developed over a decade or more.
Does a stent fix the underlying heart disease? No. A stent opens one narrowed or blocked artery and can be lifesaving during a heart attack, but atherosclerosis affects the whole arterial system. Medications, risk-factor control and lifestyle changes are needed to treat the underlying disease.
What is endothelial dysfunction? It is impaired function of the thin cell layer lining the arteries. Healthy endothelium keeps vessels relaxed and resistant to clotting and inflammation. Dysfunction is one of the earliest steps in atherosclerosis and often improves with exercise, not smoking, and blood pressure and cholesterol control.
Can atherosclerosis be reversed after stent surgery? Partly. Intensive cholesterol lowering can modestly reduce plaque volume and make plaques more stable and less likely to rupture. Endothelial function can improve within weeks to months with lifestyle changes. Discuss your targets with your cardiologist.
