Why LTC Cardiac Readmission Rates Peak in Year One
#LTC, #Senior Care, #Readmissions, #Heart Failure, #Care Transitions, #Medication Reconciliation
Every director of care knows the pattern. A resident returns from hospital after a heart failure admission or a heart attack, settles back into their room, and within weeks is on the way back out in an ambulance. Sometimes it's the same problem. Often it's something that looks unrelated: a fall, confusion, a chest infection, dehydration.
The short answer: cardiac readmissions from long-term care are front-loaded. About one in five older adults discharged from hospital is readmitted within 30 days, and nearly one in four post-acute nursing facility residents is. Risk then stays elevated through the first year because of post-hospital vulnerability, medication changes, fluid shifts, deconditioning and poor nutrition, most of which homes can monitor.
That last point is the hopeful one. Much of what drives year-one readmissions happens inside the home, on a timeline staff can see.
How common are readmissions after a cardiac hospital stay?
The landmark US analysis of Medicare claims found that 19.6% of beneficiaries discharged from hospital were rehospitalised within 30 days, with heart failure among the most common causes (Jencks et al., NEJM, 2009). For patients discharged to skilled nursing facilities, the 30-day readmission rate was 23.5% (Mor et al., Health Affairs, 2010).
Canadian data follow the same shape, and heart failure consistently sits near the top of readmission lists. The reason is not only the heart. Residents in long-term care typically combine advanced age, multiple chronic conditions, frailty and complex medication regimens, so any cardiac hospitalisation lands on a system with very little reserve.
After a heart attack specifically, about 18% of survivors have another heart attack, stroke or cardiovascular death within the first year (Jernberg et al., European Heart Journal, 2015). In older, frailer populations, that risk is higher.
Why risk is highest right after discharge: post-hospital syndrome
In 2013, Harlan Krumholz described post-hospital syndrome: a period of generalised, transient vulnerability after discharge that goes well beyond the original diagnosis (NEJM, 2013). Hospitalisation disrupts sleep, causes rapid muscle loss, interrupts eating and often involves delirium, pain and new medications. A resident returns weaker, more confused and more fragile than before, even if the heart condition itself has been treated well.
That explains why so many readmissions are for something other than the original problem. It also points to where prevention lives: in the first days and weeks back home, when sleep, strength, food intake and medications are being re-established.
The five drivers LTC teams can actually monitor
1. Medication changes. Hospital discharge frequently brings new drugs, stopped drugs and changed doses. Discrepancies between the discharge list and what is actually given are common, and in cardiac residents they often involve diuretics, anticoagulants and blood pressure medications, the classes most likely to cause harm when wrong.
2. Fluid balance. Heart failure residents live on a narrow margin between congestion and dehydration. Too little diuretic and fluid accumulates; too much, particularly with poor oral intake, leads to dehydration, kidney injury, falls and confusion.
3. Deconditioning. Days in bed cost older adults muscle they may never fully regain. Weakness increases falls, and falls are a frequent route back to hospital.
4. Nutrition and hydration. Appetite often drops after hospitalisation, and cardiac diets restricted in salt can make food less appealing still. Poor intake undermines recovery and interacts with diuretics and blood pressure drugs (see senior cardiac nutrition depletion).
5. Early warning signs. Weight gain, new swelling, breathlessness when lying flat, reduced urine output and new confusion often precede a readmission by days. Homes that catch them early can act with the physician before a crisis.
Five safeguards for the first 30 days, and the rest of year one
Evidence from care transition programmes shows these problems respond to structure. In one randomised study, a care transitions intervention reduced 30-day readmissions from 11.9% to 8.3% (Coleman et al., Archives of Internal Medicine, 2006).
Reconcile medications within 72 hours of return, with the consultant pharmacist, focusing on diuretics, anticoagulants, antiplatelets and any drug started or stopped in hospital.
Weigh heart failure residents daily, at the same time and on the same scale, with a clear threshold for calling the physician, commonly a gain of more than 2 kg over two to three days, or as directed.
Book early follow-up. Heart failure guidelines generally recommend follow-up within 7 to 14 days of discharge.
Screen nutrition and hydration on return and weekly for the first month, including appetite, fluid intake and weight trends.
Rebuild mobility deliberately, with a daily walking or activity plan to counter deconditioning and reduce falls.
After the first month, the same safeguards continue at a lower intensity for the rest of the year: monthly medication and supplement review, weight and blood pressure trends, and a standing plan for symptoms that should trigger a call.
How Healos™ supports LTC homes
Healos™ works with long-term care and senior care organisations as complementary support to the existing care team, not a replacement for physicians, nurses or pharmacists.
For LTC partners, that includes a medication-first review that flags predictable drug-nutrient effects and supplement interactions for the consultant pharmacist; structured resident summaries built on the H-Index, a communication tool rather than a diagnostic test; adherence and routine prompts that support staff rather than adding to their workload; and bilingual English and Chinese materials for residents and families. Nutritional products are Health Canada NPN-licensed and positioned as complementary support.
Homes interested in testing this approach can request an LTC pilot proposal, typically starting with a small group of residents with recent cardiac admissions.
The bottom line
Cardiac readmissions from long-term care aren't random. They cluster in the weeks after discharge and stay elevated through the first year, driven by vulnerabilities that homes can see: medications, fluid, strength, food and early warning signs. A small amount of structure in the first 30 days, sustained at lower intensity for the rest of the year, is where the gains are.
Frequently asked questions
Why are cardiac readmissions so common from long-term care? Residents combine advanced age, frailty, multiple conditions and complex medications, so a cardiac hospitalisation leaves little reserve. Post-hospital vulnerability, medication discrepancies, fluid imbalance, deconditioning and poor nutrition all contribute, especially in the first 30 days.
What is post-hospital syndrome? Post-hospital syndrome is a transient period of generalised vulnerability after discharge, described by Harlan Krumholz in 2013. Disrupted sleep, muscle loss, poor nutrition, delirium and new medications leave patients at higher risk of readmission for many different reasons, not only the original diagnosis.
How can LTC homes reduce heart failure readmissions? Key steps include medication reconciliation within 72 hours of return, daily weights with a clear call threshold, early follow-up within 7 to 14 days, weekly nutrition and hydration screening in the first month, and a deliberate mobility plan.
How soon should follow-up happen after a cardiac discharge? Heart failure guidelines generally recommend a follow-up visit within 7 to 14 days of discharge. Follow the discharging team's specific instructions for each resident.
Does Healos™ replace the LTC care team? No. Healos™ provides complementary support, including medication-first drug-nutrient screening, structured resident summaries and bilingual materials, while clinical decisions remain with physicians, nurses and pharmacists.
