A post-cardiac patient hands you a bottle of CoQ10 and asks the question every pharmacist hears weekly: "It's Health Canada approved, so it's fine, right?"
The honest answer is "partly". The eight-digit number on the label tells you a great deal about what the product is and what it's licensed to claim. It tells you nothing about whether it belongs in this particular patient's regimen, alongside their warfarin, their statin and their DAPT.
The short answer: an NPN is an eight-digit Natural Product Number showing that Health Canada has licensed a natural health product for a specific recommended use, dose and set of cautions after reviewing safety, efficacy and quality. It confirms what is licensed. It does not guarantee the product suits a particular cardiac patient, that it is free of interactions with their medications, or that marketing beyond the licence is permitted.
What an NPN actually certifies
Since the Natural Health Products Regulations (SOR/2003-196) came into force on January 1, 2004, natural health products sold in Canada have required a product licence. A licensed product carries an eight-digit NPN, or a DIN-HM for homeopathic medicines.
Behind that number sit several things a pharmacist can rely on:
The medicinal ingredients and their amounts, including the source material.
The recommended use or purpose, meaning the specific health claim Health Canada authorised.
The recommended dose, duration and route of administration.
Cautions, warnings, contraindications and known adverse reactions that must appear on the label.
Manufacturing under a site licence, with Good Manufacturing Practices requirements for the sites that make, package, label or import the product.
In other words, an NPN is strong evidence that the product in the bottle is what the label says, made to a standard, and making claims Health Canada has reviewed.
How Health Canada weighs the evidence
Health Canada takes a risk-based approach. Many common ingredients are licensed against monographs: pre-cleared documents that set out acceptable ingredients, doses, claims and cautions. A product that matches a monograph exactly can be licensed on that basis. Products with higher-risk claims, novel ingredients or doses outside a monograph need more supporting evidence, and the evidence required rises with the seriousness of the claim.
This matters for counselling. A fish oil or CoQ10 product licensed against a monograph is making a claim that sits within an established framework. A product with an unusual ingredient or an ambitious claim deserves a closer look at exactly what was licensed, and on what basis.
What an NPN does not tell you
This is where most patient confusion lives. An NPN does not:
Screen for interactions with a specific patient's medications. Label cautions are general. Whether CoQ10 matters for this patient's INR, or fish oil for this patient's bleeding risk on DAPT, is a professional judgement (see five drug-nutrient interactions).
Authorise claims beyond the licence. A website, social media post or sales pitch that goes further than the licensed recommended use isn't covered by the NPN.
Rank products against each other. Licensing doesn't establish that one brand is better absorbed or more effective than another.
Replace evidence for a disease outcome. A claim such as "helps support cardiovascular health" is not evidence that a product reduces heart attacks or strokes.
Confirm that the dose suits a frail, older or renally impaired patient. Magnesium in reduced kidney function is the classic example.
Stay current by itself. Licences can be amended, suspended or cancelled, so a quick database check is more reliable than the label alone.
① 针对特定患者的药物筛查相互作用——标签注意事项是通用的,CoQ10 是否影响这位患者的 INR、鱼油是否增加其 DAPT 下的出血风险,是专业判断;
② 授权超出许可的宣称——超出推荐用途的网站、社交媒体或销售话术不在 NPN 覆盖范围内;
③ 对产品进行排名——许可不能证明某品牌吸收更好或更有效;
④ 替代疾病结局证据——「帮助支持心血管健康」不等于证明能减少心梗或卒中;
⑤ 确认剂量适合衰弱、高龄或肾功能受损的患者——肾功能下降时的镁是典型例子;
How to verify an NPN in 60 seconds
The Licensed Natural Health Products Database (LNHPD) is public and searchable by NPN, product name, company or ingredient. A quick check at the counter answers most questions:
Enter the eight-digit NPN and confirm the product name and company match the bottle.
Read the recommended use. Is the patient buying it for that purpose, or for something the licence doesn't cover?
Compare the dose on the licence with how the patient is actually taking it.
Review cautions and contraindications, especially anything about anticoagulants, pregnancy, kidney disease or surgery.
Glance at the non-medicinal ingredients for allergens or, for patients on sodium or potassium limits, relevant salts.
If the number doesn't appear, or the details don't match, that is a conversation worth having before the patient continues.
① 输入 8 位 NPN,确认产品名和公司与瓶身一致;
③ 对比许可剂量与患者实际服用方式;
④ 查看注意事项和禁忌,尤其涉及抗凝药、妊娠、肾病或手术的内容;
Where marketing crosses the line
An NPN licenses a claim, not a marketing campaign. Advertising for a natural health product must be consistent with its licensed recommended use, and the Food and Drugs Act (section 3 and Schedule A.1) restricts advertising to the general public that presents a product as a treatment, preventative or cure for listed serious diseases.
In cardiovascular supplements, the familiar overreaches are easy to spot once you look for them: "treats heart disease", "prevents a second heart attack", "unblocks arteries", "repairs brain cells after stroke", "replaces your statin". None of these belong on a label or in marketing for a natural health product, whatever its NPN. When patients repeat these claims at the counter, it is often the most useful moment to reset expectations: supplements can offer complementary support within their licensed use, while prescribed therapy does the heavy lifting.
What has changed recently
Two developments are worth knowing about.
Labelling. Health Canada amended natural health product labelling requirements in 2022, introducing a standardised Product Facts table and clearer allergen and contact information, with phased transition periods for existing products. Check Health Canada's current guidance for the dates that apply to a given product.
Enforcement. The Budget Implementation Act, 2023, No. 1 extended powers from the Protecting Canadians from Unsafe Drugs Act, often called Vanessa's Law, to natural health products. That gives Health Canada authority to order recalls and label changes and exposes companies to substantially higher penalties.
For pharmacists, the practical point is unchanged but reinforced: suspected adverse reactions to natural health products should be reported to Canada Vigilance, just as they would be for prescription drugs.
How Healos™ applies NPN discipline
Healos™ holds itself to the same standard pharmacists apply at the counter. Its nutritional products are Health Canada NPN-licensed and described only within their authorised claims and positioned as complementary support, never as treatment, cure or prevention of any disease. Molecular hydrogen water, for example, is described as providing antioxidant support, not as a treatment for ischaemia or stroke. The H-Index is described consistently as a pre-visit communication tool, not a diagnostic test. And where an ingredient's evidence or regulatory status is still developing, Healos™ avoids making specific efficacy claims until that work is complete.
Every programme also starts with the patient's medication list, so interaction questions reach the pharmacist before a product reaches the patient.
The bottom line
An NPN answers "is this product what it says, and is its claim licensed?" It doesn't answer "is this right for this patient?" That second question belongs to the pharmacist, and in post-cardiac patients on antithrombotics, statins and diuretics, it's the one that matters most.
Frequently asked questions
What does an NPN number mean on a supplement? An NPN is an eight-digit Natural Product Number showing that Health Canada has licensed the product under the Natural Health Products Regulations for a specific recommended use, dose and set of cautions, after reviewing its safety, efficacy and quality.
How do I check if a supplement is licensed in Canada? Search the product's NPN in Health Canada's Licensed Natural Health Products Database (LNHPD). Confirm the product name and company, then review the licensed recommended use, dose, cautions and non-medicinal ingredients.
Can a supplement claim to treat heart disease in Canada? No. Claims must match the product's licensed recommended use, and the Food and Drugs Act restricts advertising to the public that presents products as treatments, preventatives or cures for listed serious diseases. Wording such as "helps support cardiovascular health" is only permitted where it is licensed.
Does an NPN mean a supplement is safe with my heart medications? Not necessarily. An NPN confirms general safety at the licensed dose, but it doesn't screen for interactions with your specific medications, such as warfarin, antiplatelets, statins or diuretics. Ask your pharmacist before starting any supplement.
What changed in Canadian natural health product rules recently? Health Canada introduced new labelling requirements in 2022, including a Product Facts table, with phased transitions, and in 2023 extended Vanessa's Law powers such as mandatory recalls and label changes to natural health products.
